摘要
目的:探讨贯通口底切除术治疗舌癌的疗效及对患者血清血管内皮生长因子(VEGF)、催乳素(PRL)水平的影响。方法:纳入64例舌癌患者,随机分为2组(n=32)。对照组行舌颌颈联合切除术,实验组行贯通口底切除术。比较2组手术满意度、并发症发生情况、手术前后血清VEGF、PRL水平、生存质量(KPS)评分及生存率。结果:实验组外形、开口度、语音清晰、咀嚼吞咽功能恢复满意度高于对照组(P<0.05);实验组与对照组并发症发生率分别9.38%和21.88%(P<0.05);术后7、14d,2组血清VEGF、PRL水平较术前明显下降(P<0.05),实验组低于对照组(P<0.05);术后3、6个月,2组KPS评分均较术前升高(P<0.05),实验组高于对照组(P<0.05);实验组术后3年总生存率90.63%、无瘤生存率50.00%,高于对照组67.74%和25.81%(P<0.05)。结论:贯通口底切除术治疗舌癌可显著提高手术满意度,降低并发症发生率,还可降低血清VEGF、PRL水平,改善生存质量,提高生存率。
Objective:To investigate the effects of trans-oral resection for tongue cancer and its effects on serum VEGF and PRL lev-els.Methods:64 patients with tongue cancer were included and randomly divided into 2 groups(n=32).The patients in control group underwent tongue,jaw and neck combined radical surgery,and those in the experimental group underwent trans-oral resection.The sur-gical satisfaction,the complications,the levels of serum VEGF and PRL,quality of life scores(KPS)and survival rate after surgery were compared between the 2 groups.Results:The satisfaction of the appearance,mouth opening degree,voice clearness and recovery of chewing and swallowing function of the experimental group were higher than those of the control group(P<0.05);the complication rate in the experimental and control group was 6.25%and 28.13%respectively(P<0.05);7 and 14 days after operation,the level of serum VEGF and PRL in the 2 groups were dicreased(P<0.05)and that of experimental group was lower than that of the control group(P<0.05);at3 and 6 months after surgery,the KPS of both groups were increased(P<0.05),and that of the experimental group was higher than that of the control group(P<0.05);the 3 year overall survival rate of the experimental group was 90.63%and the tumor-free survival rate was 50.00%,those of the control group was 67.74%and 25.81%respectively(P<0.05).Conclusion:Trans-oral resection of tongue cancer can improve the satisfaction of surgery,and decrease the complications.It can also reduce the lev-els of serum VEGF and PRL,improve the quality of life and increase the survival rate.
