摘要
目的探究血清高迁移率族蛋白B1(HMGB1)、脑钠尿肽(BNP)水平与新生儿持续性肺动脉高压(PPHN)的关系。方法选取2016年12月—2019年10月保定市第一中心医院收治的46例PPHN患儿作为PPHN组,选取同期住院的46例非持续性肺动脉高压新生儿作为对照组。采用酶联免疫吸附法(ELISA)检测血清HMGB1、BNP、肿瘤坏死因子-α(TNF-α)、内皮素-1(ET-1)水平,以Pearson法分析PPHN患儿血清HMGB1、BNP水平与TNF-α、ET-1的相关性,采用受试者工作特征曲线(ROC)评价血清HMGB1、BNP对PPHN的诊断价值。结果PPHN组患儿血清HMGB1、BNP、TNF-α、ET-1水平均明显高于对照组(P均<0.05);PPHN患儿血清HMGB1、BNP水平与TNF-α、ET-1均呈正相关(P均<0.05);血清HMGB1、BNP对PPHN诊断的曲线下面积(AUC)为0.840,0.796,截断值分别为7.62 ng/mL、0.63 ng/mL,相应灵敏度分别为76.1%,73.9%,特异度分别为84.8%,87.0%;两者联合诊断PPHN的AUC为0.895,其灵敏度、特异度分别为89.1%,87.0%。结论HMGB1、BNP在PPHN患儿血清中均显著升高,两者可能与TNF-α、ET-1协同促进PPHN发展,两者联合检测可有效提高对PPHN的诊断价值,有利于筛查PPHN患儿。
Objective It is to investigate the relationships between serum high mobility group protein box 1(HMGB1),brain natriuretic peptide(BNP)and persistent pulmonary hypertension of newborn(PPHN).Methods A total of 46 children with PPHN admitted to the First Central Hospital of Baoding from December 2016 to January 2020 were selected for the study,named as PPHN group;another 46 children with non persistent pulmonary hypertension in the same period were selected as the control group.The levels of serum HMGB1,BNP,tumor necrosis factor-α(TNF-α)and endothelin-1(ET-1)were measured by enzyme-linked immunosorbent assay(ELISA);Pearson method was used to analyze the correlation between serum HMGB1,BNP levels and TNF-α,ET-1 in PPHN children;and the diagnostic value of serum HMGB1 and BNP in PPHN was evaluated by receiver operating characteristic curve(ROC).Results The levels of serum HMGB1,BNP,TNF-αand ET-1 in PPHN group were significantly higher than those in the control group(P<0.05);the levels of serum HMGB1 and BNP of PPHN children were positively correlated with TNF-αand ET-1(P<0.05);the area under curve(AUC)of serum HMGB1 and BNP in diagnosis of PPHN was 0.840 and 0.796,the cutoff value was 7.62 ng/mL and 0.63 ng/mL respectively,the corresponding sensitivity was 76.1%and 73.9%,and the specificity was 84.8%and 87.0%,respectively;meanwhile,the AUC of the combination in the diagnosis of PPHN was 0.895,and the sensitivity and specificity were 89.1%and 87.0%,respectively.Conclusion HMGB1 and BNP are significantly increased in PPHN children’s serum,which may act with TNF-αand ET-1,and then affect the development of PPHN.The combination of HMGB1 and BNP can effectively improve the diagnostic value of PPHN and help to screen PPHN children.
