摘要
目的探讨ICU机械通气患者发生呼吸机相关肺炎(VAP)的影响因素及病原菌分布情况。方法选择2017年5月至2019年6月我院ICU行机械通气的患者405例,分析可能影响VAP发生的因素及病原菌分布情况。结果405例机械通气发生VAP 95例,VAP发生率为23.46%。共检出病原菌121株,其中革兰阴性菌75株占61.98%,革兰阳性菌41株占33.88%,真菌5株占4.13%。APACHE II评分、平卧位、抑酸剂、血清白蛋白以及留置胃管时间≥20 d,为影响ICU机械通气发生VAP的独立危险因素。结论ICU机械通气VAP发生率较高,主要病原菌为革兰阴性菌,影响VAP发生的因素较为复杂,应针对相关危险因素采取相应对策,以减少VAP的发生。
Objective To explore the influencing factors of occurrence of ventilator associated pneumonia(VAP)in patients with mechanical ventilation in ICU and distribution of pathogen.Methods To select 405 cases of patients who underwent mechanical ventilation in the ICU of our hospital from May 2017 to June 2019 and analyze the factors that may affect the occurrence of VAP and the distribution of pathogens.Results There were 95 cases of VAP occurred in 405 cases of mechanical ventilation,and the incidence rate of VAP was 23.46%.A total of 121 pathogenic bacteria were detected,of which 75 gram-negative bacteria accounted for 61.98%,41 gram-positive bacteria accounted for 33.88%,and 5 fungi accounted for 4.13%.APACHE II score,horizontal position,acid inhibitors,serum albumin,and time of indwelling gastric tube≥20 d were the independent risk factors that affected the occurrence of VAP in ICU mechanical ventilation.Conclusion The mechanical ventilation in ICU had a high incidence of VAP.The main pathogen was Gram-negative bacteria.The factors affecting the occurrence of VAP were more complicated.So the corresponding measures should be taken to reduce the occurrence of VAP.
