摘要
目的对研究腹膜透析患者中医证型的文献进行特征分析,总结研究现状及趋势,以指导临床治疗和科学研究。方法检索中国知识资源总库(CNKI)、中文科技期刊数据库(维普网)、中国学术期刊数据库(万方数据),对纳入文献的基本信息、中医证候参考标准、临床试验研究方式、相关并发症、化验室指标等进行统计分析。结果共纳入文献39篇,其中硕士毕业论文20篇(占51.3%),研究中医证型与合并症、化验指标相关性的文献36篇(占92.3%)。腹膜透析患者中,本虚证以脾肾气虚、脾肾阳虚、气阴两虚较多,标实证以湿浊、湿热、血瘀证为多。涉及营养状态、透析龄、腹膜转运类型的文献较多,达26篇(占72.2%)。观察的疗效指标主要为血红蛋白、血白蛋白、尿素清除指数、C反应蛋白、甲状旁腺激素。结论腹膜透析患者中医证型与病症、化验室指标的相关性比较受研究者重视,尤其以营养状态、血红蛋白、血白蛋白为主。另外,期待大样本的腹膜透析患者中医证候类型变化规律的研究。
Objective To analyze the characteristics of literature about TCM syndrome types of patients with peritoneal dialysis;To summarize the current research status and trend;To guide clinical treatment and scientific research. Methods China Knowledge Resources Database(CNKI), Chinese Sci-tech Periodicals Database(VIP), China Academic Journal Database(Wanfang Date) were retrieved. The basic information of the included literature, reference criteria for TCM syndromes, research methods of clinical trials, relevant complications, and laboratory indicators were analyzed. Results Totally 39 articles were included, among which 20 were the graduate thesis(51.3%), and 36 articles(92.3%) were about the correlation between TCM syndrome types and complications and laboratory indicators. Among the patients with peritoneal dialysis, essence deficiency syndrome was mainly spleen-kidney qi deficiency, spleen-kidney yang deficiency, and qi-yin deficiency;superficiality excess syndrome was mainly dampness and turbidity, dampness and heat, and blood stasis. There were many articles on nutritional status, age of dialysis, and types of peritoneal transport, reaching 26(72.2%). The main indexes of efficacy were hemoglobin, albumin, urea removal index, C-reactive protein, and parathyroid hormone. Conclusion The correlation between TCM syndrome types of patients with peritoneal dialysis, the disease and laboratory indicators has attracted more attention from researchers, especially in terms of nutritional status, hemoglobin and albumin. In addition, research on changes laws of TCM syndrome types of large-sample peritoneal dialysis patients is expected.
