摘要
目的比较玻璃体切除术与玻璃体切除术联合内界膜剥除术治疗外伤性黄斑孔(TMH)的效果。方法前瞻性病例对照研究。以2016年1月至2019年1月在郑州市第二人民医院接受玻璃体切除术治疗的外伤性黄斑孔64例(64只眼)为研究对象,按随机数字表法分为两组,每组各32例。其中对照组行玻璃体切除术治疗,临床试验组行玻璃体切除联合内界膜剥除术治疗。比较两组术后视力及黄斑孔情况。术后随访6个月。结果术后两组视力均有不同程度的恢复(P<0.05)。术后6个月试验组视力(logMAR)高于对照组(t=3.988,P=0.000),试验组视力提高率高于对照组(χ^2=4.267,P=0.039)。术后6个月对照组黄斑孔闭合率为75.00%,试验组为93.75%,差异无统计学意义(χ^2=1.896,P=0.168)。两组患者手术后黄斑孔直径逐月缩小(P<0.05),且试验组患者术后黄斑孔直径均小于对照组(F时点=9483.998、F交互=119.806、F组间=94.454,P<0.05)。结论玻璃体切除联合内界膜剥离术能够更有效地治疗外伤性黄斑孔。
Objective To compare the efficacy of vitrectomy and vitrectomy combined with internal limiting membrane peeling for traumatic macular hole(TMH).Methods This was a prospective case-control study.A total of 64 eyes of 64 cases of traumatic macular hole who received vitrectomy from Jan.2016 to Jan.2019 in the Second People’s Hospital of Zhengzhou were selected.All cases were divided into two groups based on the random number table method,with 32 cases in each group.The control group underwent vitrectomy.The clinical trial group underwent vitrectomy combined with the inner limiting membrane peeling.The visual acuity and macular hole between the two groups were compared.The follow-up time was 6 months.Results After 6 months of follow-up,the visual acuity of both groups improved to different degrees(P<0.05).The visual acuity(logMAR)of the trial group at 6 months postoperatively was higher than that of the control group(t=3.988,P=0.000),and the rate of vision improvement of the trial group was higher than that of the control group(χ^2=4.267,P=0.039)at 6 months after surgery.The closure rate of macular hole was 75.00%in the control group and 93.75%in the trial group.But there was no significant difference between the two groups at 6 months after surgery(χ^2=1.896,P=0.168).The diameter of macular hole decreased monthly after surgery in both groups(P<0.05),and the diameter of macular hole after surgery in the trial group was smaller than that in the control group(Ftime=9483.998,Finteractive=119.806,Fgroups=94.454,P<0.05).Conclusion Vitrectomy combined with internal limiting membrane peeling is more effective for the treament of traumatic macular hole.
