迷走神经刺激对癫痫合并抑郁患者血清细胞因子及神经递质的影响 被引量:11

Effects of vagus nerve stimulation on serum cytokines and neurotransmitters in epilepsy patients with depression

摘要 目的 探讨迷走神经刺激(VNS)对癫痫合并抑郁患者血清细胞因子及神经递质的影响.方法 选取2015年3月至2018年3月湖南省脑科医院收治的13例癫痫合并抑郁并行VNS手术患者,比较其术前、术后6个月及12个月时手术疗效、抑郁程度、血清细胞因子及神经递质改变情况.结果 VNS术后6、12个月时患者癫痫治疗有效率分别为61.5% (8/13)、76.9% (10/13).术后6个月及12个月,患者汉密顿抑郁量表(HAMD)、抑郁自评量表(SDS)分值均较术前明显降低(P<0.05或P<0.01).术后6个月时血清5-羟色胺(5-HT)水平较术前明显升高,IL-1α、IL-6、TNF-α较术前明显降低,差异有统计学意义(P <0.05或P<0.01);术后12个月时血清5-HT、多巴胺(DA)较术前明显升高,IL-1 α、IL-2、IL-6、TNF-α较术前明显降低,差异有统计学意义(P<0.05或P<0.01).结论 VNS可能通过调节神经递质及血清细胞因子水平,使癫痫及抑郁均得到不同程度改善. Objective To investigate the effect of vagus nerve stimulation(VNS)on serum cytokines and neurotransmitters in epilepsy patients with depression.Methods From March 2015 to 2018,13 patients with epilepsy combined with depression undergoing VNS surgery in Brain Hospital of Hunan Province were selected as the research objects.The surgical efficacy,depression degree,serum cytokines and neurotransmitter changes before operation,6 months and 12 months after operation were compared.Results The effective rate of epilepsy treatment at 6 and 12 months after VNS were 61.5%(8/13)and 76.9%(10/13),respectively.After 6 months and 12 months,Hamiton Depression scale(HAMD)and self-rating depression scale(SDS)scores were significantly lower than those before operation(P<0.05 or P<0.01).At 6 months after operation,serum 5-hydroxytryptamine(5-HT)level was significantly higher than that before operation,while serum interleukin(IL)-1α,IL-6 and tumor necrosis factorα(TNF-α)were significantly lower than those before operation(P<0.05 or P<0.01).At 12 months after operation,5-HT and dopamine(DA)were significantly increased,while IL-1α,IL-2,IL-6 and TNF-αwere significantly decreased(P<0.05 or P<0.01).Conclusions VNS may can improve epilepsy and depression by regulating the level of neurotransmitter and serum cytokines.
