摘要 目的探讨失效模式与效应分析(FMEA)在门诊血液标本采集中的应用效果。方法采用FMEA对2019年2―4月门诊采集血液标本流程中的不良事件进行分析,查找失效原因,计算风险优先指数(RPN),依据RPN制定针对性的改进措施,并将改进措施应用于门诊血液标本采集流程中。对比分析改进措施实施前后门诊血液标本采集中风险优先指数及不良事件的发生率。结果FMEA改进措施实施后门诊血液标本采集的RPN显著低于改进前;不良事件的发生率由改进前的0.005%下降到0.003%;差异具有统计学意义(p<0.05)。结论FMEA在门诊血液标本采集中的有效应用,能够显著降低RPN及不良事件发生率。 Objective To explore the application effect of failure mode and effects analysis(FMEA)in blood specimen collection in outpatient department.Methods FMEA was used to analyze the adverse events in the blood specimen collection process from February to April 2019,find out the causes of failure,calculate the risk priority number(RPN),formulate targeted improvement measures based on RPN,and apply the improvement measures to the blood specimen collection process in outpatient department.The RPN and the incidence rate of adverse events during the blood specimen collection in outpatient department before and after the implementation of the improvement measures were compared.Results After the implementation of FMEA management measures,the RPN of blood specimen collection in outpatient department was significantly lower than that before improvement;the incidence rate of adverse events decreased from 0.005%before improvement to 0.003%;the difference was statistically significant(p<0.05).Conclusion The effective application of FMEA in outpatient blood specimen collection can significantly reduce the RPN and incidence rate of adverse events.
