摘要
目的:从菌群总体角度分析比较不同严重程度宫腔粘连(IUA)患者与健康育龄期女性的子宫内膜菌群的差异。方法:采集20例轻度IUA患者(IUA-L)、26例中重度IUA(IUA-M/H)及21例子宫内膜正常女性(对照组)的子宫内膜组织,对样本进行细菌基因组DNA提取、16S rRNA V4区基因扩增及采用Illumina高通量测序,比较3组子宫内膜菌群物种丰度结构及多样性。结果:在菌门水平,所有研究对象的子宫内膜菌群主要由变形菌门、厚壁菌门、拟杆菌门、放线菌门组成,三个组中变形菌门高达67.16%。在菌属水平,IUA组子宫内膜克雷伯氏菌属、希瓦氏菌属、乳酸杆菌属的相对丰度较对照组增加(31.44%、15.50%、8.77%,9.51%、15.32%、4.53%,7.11%、16.00%、6.95%)。IUA-L组和IUA-M/H组的Alpha多样组明显低于对照组(P=0.012,P=0.030);轻度IUA组与对照组的子宫内膜菌群构成在总体上可区分(P=0.022)。结论:IUA患者与子宫内膜正常女性的子宫内膜菌群构成存在差异,主要改变优势菌群的组成比例,且分布欠均匀、群落结构相似度较低。IUA发病可能与子宫内膜菌群改变有关,维持子宫腔内微生态平衡有利于子宫内膜损伤后修复。
Objective:From the perspective of overall flora analysis,the endometrial microbiota composition of patients with different degrees of intrauterine adhesions and healthy women of childbearing age were compared.Methods:Endometrial tissue samples were collected from 20 patients with mild IUA(Group IUA-L),26 patients with moderate and severe IUA(Group IUA-M/H)and 21 women with normal endometrium.The bacterial genomic DNA extraction was followed by the 16s rRNA V4 gene amplification and Illumina high-throughput sequence,which were used to compare the species abundance structure and diversity of endometrial microbiota among the three groups.Results:At the phylum level,the endometrial microbiota of all subjects was mainly composed of Proteobacteria,Firmicutes,Bacteroidetes,Actinobacteria,of which Proteobacteria was as high as 67.16%.At the genus level,the relative abundance of Klebsiella,Shewanella and Lactobacillus in IUA patients were higher than those in the control group(31.44%、15.50%vs 8.77%,9.51%、15.32%vs 4.53%,7.11%、16.00%vs 6.95%).The Alpha diversity of group IUA-L and group IUA-M/H decreased significantly(P=0.012,P=0.030),and the composition of endometrial microbiota could be distinguished between group IUA-L and group C(P=0.022).Conclusion:There were differences in the composition of endometrial microbiota between IUA patients and healthy control cohort,mainly changing the proportion of dominant microbiota,uneven distribution and low similarity of community structure.The pathogenesis of IUA may be related to the changes of endometrial microbiota,and the maintenance of microecological balance in the uterine cavity is beneficial to the repair of endometrium after injury.
