摘要
目的探讨术前磁共振病灶环形强化征与乳腺癌术后早期复发的相关性。方法回顾性分析2012年1月至2015年1月我院77例乳腺癌患者的术前磁共振以及临床病理资料,依据术前磁共振病灶是否存在环形强化征象,将患者分为环形强化阳性组与阴性组,比较两组患者临床病理资料以及术后早期复发的相关性。结果 77例乳腺癌患者中,环形强化阳性患者23例、阴性54例。与环形强化阴性患者比较,阳性组T分级与N分级更高、三阴性乳腺癌占比更多、Ki-67水平更高。生存分析显示,环形强化阳性患者术后36个月无瘤生存率低于阴性组,分别为43.5%、96.4%(Log-rankχ~2=32.125,P<0.001)。COX单因素分析结果显示,肿瘤的T分期、N分期、分子亚型、环形强化是乳腺癌术后早期复发的相关因素(均P<0.05);COX多因素分析结果显示,肿瘤的N分期(HR=5.195,P=0.004)、环形强化(HR=9.426,P=0.024)是乳腺癌术后早期复发的独立危险因素。结论术前磁共振病灶环形强化征与乳腺癌多种恶性临床病理特征具有很大的相关性,是预测患者术后早期复发的一项特征性影像学征象。
Objective To investigate the correlation between ring-enhancement at preoperative MR imaging and early postoperative recurrence of breast cancer.Methods A retrospective analysis was from preoperative MR imaging and clinicopathological data of 77 patients with breast cancer in our hospital from January 2012 to January 2015.The patients were divided into ring-enhancement positive group and negative group,and the correlation of clinicopathological data between the two groups was compared.Results In 77 cases of breast cancer,23 cases were positive for ring enhancement and 54 cases were negative.Compared with ring-negative patients,the T grade was higher than that of N and three negative breast cancer,Ki-67 level higher.The survival analysis showed that disease free survival of 36 months after operation was lower than that of negative group,which was 43.5%、96.4%(Log-rankχ~2=32.125,P<0.001).Univariate analysis of COX showed that T stage,N stage,molecular subtype and ring-enhancement were related factors for early recurrence of breast cancer after operation (P<0.05).COX multivariate analysis showed that the N staging (HR=5.195,P=0.004),ring-enhancement(HR=9.426,P=0.024)were independent risk factors for the early recurrence of breast cancer.Conclusion Ring-enhancement at preoperative MR imaging is closely related to many malignant clinicopathological features of breast cancer,and is a characteristic imaging sign for predicting the early postoperative recurrence of breast cancer patients.
