摘要
目的:研究肺结核致胸腔积液诊疗中彩色多普勒超声的应用效果。方法:研究对象为2016年1月-2018年12月入院的260例肺结核致胸腔积液患者,所有患者均行X线与彩色多普勒超声检查,以胸腔穿刺术病理结果为诊断金标准,比较两种方法肺结核致胸腔积液诊断准确率;同时,按照结核性胸腔积液超声分型予以针对性治疗,观察并记录不同超声分型患者总有效率。结果:X线肺结核致胸腔积液诊断准确率为90.77%(236/260),超声肺结核致胸腔积液诊断准确率为99.62%(259/260),差异有统计学意义(χ^2=14.732,P=0.000)。260例肺结核致胸腔积液患者超声分型及总有效率:Ⅰ型38例,总有效率为100%;Ⅱ型47例,总有效率为97.87%;ⅢA型55例,总有效率为96.36%;ⅢB型72例,总有效率为91.67%;ⅢC型18例,总有效率为88.89%;Ⅳ型30例,总有效率为86.67%。结论:肺结核致胸腔积液检查中彩色多普勒超声的诊断准确率高于X线,且通过超声分型开展针对性治疗可获取显著效果。
Objective:To study the application of color Doppler ultrasound in the diagnosis and treatment of pleural effusion caused by tuberculosis.Method:A total of 260 patients with pleural effusion caused by tuberculosis admitted from January 2016 to December 2018 were studied.All patients were examined by X-ray and color Doppler ultrasound.The diagnostic accuracy of pleural effusion caused by tuberculosis by the two methods was compared based on the pathological results of thoracentesis as the diagnostic gold standard.At the same time,specific treatment was conducted according to the ultrasound classification of tuberculous pleural effusion,and the total effective rate of patients with different ultrasound classifications was observed and recorded.Result:The diagnostic accuracy of pulmonary tuberculois-induced pleural effusion by the X-ray was 90.77%(236/260),and that by the ultrasound was 99.62%(259/260),the difference was statistically significant(χ^2=14.732,P=0.000).The ultrasonic classification and total effective rate of 260 patients with pleural effusion caused by tuberculosis:38 cases of typeⅠ,the total effective rate was 100%,47 cases of typeⅡ,the total effective rate was 97.87%,55 cases of typeⅢ,the total effective rate was 96.36%,72 cases of typeⅢB,the total effective rate was 91.67%,18 cases of typeⅢC,the total effective rate was 88.89%,30 cases of typeⅣ,the total effective rate was 86.67%.Conclusion:The diagnostic accuracy of color Doppler ultrasound is higher than that of X-ray in the examination of pleural effusion caused by pulmonary tuberculosis,and it can obtain significant effect through the targeted treatment of ultrasonic classification.
