摘要
目的:研究腹腔镜手术治疗宫外孕的效果及对血液流变学指标及hCG水平的影响。方法:选取2016年12月-2019年10月笔者所在医院宫外孕患者54例为研究对象,按照随机数字表法将其分为对照组和观察组,各27例。对照组予以开腹手术治疗,观察组予以腹腔镜手术治疗。比较两组治疗效果、手术指标、hCG水平、血液流变学指标及术后并发症发生情况。结果:观察组治疗总有效率明显高于对照组,差异有统计学意义(P<0.05)。观察组手术时间、住院时间、排气时间均优于对照组,术中出血量少于对照组,差异均有统计学意义(P<0.05)。术后两组hCG水平比较差异无统计学意义(P>0.05)。术后3个月观察组PI、RI低于对照组,Vmax、Vmin均高于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:腹腔镜手术治疗宫外孕可以提高疗效,缩短手术时间及住院时间,减少术中出血量,有利于术后恢复,更好地改善血流动力学指标,可在临床推广。
Objective:To study the effect of laparoscopic surgery on ectopic pregnancy and its influence on hemorheology index and hCG level.Method:A total of 54 patients with ectopic pregnancy in our hospital from December 2016 to October 2019 were selected as the study objects.According to the random number table method,they were divided into the control group and the observation group,with 27 cases in each group.The control group was treated by laparotomy,and the observation group was treated by laparoscopic surgery.The therapeutic effect,operation index,hCG level,hemorheology index and postoperative complications were compared between the two groups.Result:The total effective rate of treatment in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The operation time,hospitalization time and exhaust time in the observation group were all better than those in the control group,and the amount of intraoperative blood loss was less than that of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in hCG level between the two groups after surgery(P>0.05).Three months after surgery,PI and RI in the observation group were lower than those in the control group,and Vmax and Vmin were higher than those in the control group,the differences were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Laparoscopic surgery for ectopic pregnancy can improve the curative effect,shorten the operation time and hospitalization time,reduce the amount of intraoperative blood loss,which is conducive to postoperative recovery and better improve hemodynamic index,which can be promoted in clinical practice.
