摘要
目的:探讨运用红外热像评价青藤碱定点介入治疗痛风性关节炎的临床疗效。方法:选取2019年1月—2019年8月接受治疗的90例痛风性关节炎急性期湿热蕴结型患者,按照随机数字表法分为甲、乙、丙三组,每组各30例。甲组采用青藤碱联合利多卡因关节注射治疗,乙组采用曲安奈德联合利多卡因关节注射治疗,丙组单纯采用利多卡因关节注射治疗。比较三组患者治疗前后的视觉模拟评分(VAS)、红外热像差值以及临床疗效。结果:甲组治疗后的VAS评分、红外热像差值均低于乙组和丙组,差异具有统计学意义(P<0.05);甲组总有效率与乙组比较,差异无统计学意义(P>0.05),而甲组和乙组的总有效率均明显高于丙组,差异具有统计学意义(P<0.05)。结论:青藤碱关节腔内注射能有效缓解痛风性关节炎患者急性期关节肿痛的症状,且安全有效,同时运用红外热成像进行客观疗效评估,为痛风性关节炎的临床研究提供了新思路。
Objective:To evaluate the clinical efficacy of sinomenine in the treatment of gouty arthritis by infrared thermography.Methods:Ninety patients with acute spleen-induced arthritis of gouty arthritis who were treated in hospital from January 2019 to August 2019 were randomly divided into groups A,B and C according to the random number table.30 cases.Group A was treated with sinomenine combined with lidocaine joint injection.Group B was treated with joint injection of triamcinolone acetonide and lidocaine.Group C was treated with lidocaine joint injection alone.The visual analogue scale(VAS),infrared thermal aberration value and clinical efficacy of the three groups of patients before and after treatment were compared.Results:The VAS score and infrared thermography after treatment in group A were lower than those in group B and C,and the difference was statistically significant(P<0.05).The total effective rate of group A was not statistically significant compared with the total effective rate of group B.(P>0.05),and the total effective rate of group A and group B was significantly higher than that of group C,the difference was statistically significant(P<0.05).Conclusion:Intra-articular injection of sinomenine can effectively relieve the symptoms of acute joint swelling and pain in patients with gouty arthritis,and it is safe and effective.At the same time,infrared thermography is used for objective therapeutic evaluation,which provides a clinical new ideas for gouty arthritis.
