摘要
目的:探讨隔附子饼灸联合膀胱功能训练治疗脊髓损伤后尿潴留的临床疗效。方法:以2017年1月至2018年10月本院收治的60例不完全脊髓损伤尿潴留患者为研究对象,所有患者采用随机数字表法随机分为对照组和治疗组,每组30例。对照组采用基础治疗和膀胱功能训练,治疗组在对照组的基础上联合隔附子饼灸治疗,每次30min,每日1次。两组均以4周为1个疗程,治疗2个疗程后比较两组患者的临床疗效、膀胱残余尿量、尿路感染及生活自理能力(MBI评分)情况。结果:治疗组治疗总有效率(86.67%,26/30)显著优于对照组(73.33%,22/30)(P<0.05);两组患者膀胱残余尿量均显著低于治疗前(P<0.05),且治疗组膀胱残余尿量治疗8周前后差值显著低于对照组(P<0.05);治疗组患者的MBI评分治疗8周前后差值显著高于对照组(P<0.05)。在治疗8周时,治疗组患者的尿路感染改善情况显著优于对照组(P<0.05)。结论:隔附子饼灸联合膀胱功能训练能增强膀胱控尿能力,维护膀胱顺应性,促进恢复膀胱自主排尿功能,收到较好疗效。
Objective:To explore the clinical effects that aconite moxibustion combined with bladder function training for treating urinary retention after incom lete spinal cord injury.Methods:Sixty patients with incomplete spinal cord injury and urinary retention treated in our hospital from January 2017 to October 2018 were divided randomly into control group and treatment group,30 cases in each group.Both groups were treated with conventional treatment and bladder function training,while the treatment group was additionally given aconite moxibustion for once a day for 30 minutes.In both groups,4 weeks were taken as a course of treatment,and the treatment efficiency,residual urine volume,urinary tract infection,and self-care ability(MBI scores)were compared after two courses of treatment.Results:After 8 weeks of treatment,the total effective rate of the treatment group(86.67%,26/30)was significantly better than that of the control group(73.33%,22/30)(P<0.05).Bladder residual urine volume in both groups was significantly lower than that before treatment(P<0.05),the difference value of bladder residual urine volume in the treatment group before and after 8 weeks of treatment was significantly lower than that in the control group(P<0.05).The difference value of MBI scores in the treatment group before and after 8 weeks of treatment was significantly higher than that in the control group(P<0.05).The improvement of urinary tract infection in the treatment group was better than that in the control group after 8 weeks of treatment(P<0.05).Conclusion:It has been found that aconite moxibustion combined with bladder function training can enhance the bladder's ability to control urine,maintain bladder compliance,and promote the recovery of bladder's autonomic urination function,and the clinical effect is positive.
