摘要
目的回顾性分析急危重心律失常患者的动态心电图数据,分析其Lorenz RR散点图特征,探讨Lorenz RR散点图快速诊断急危重心律失常的价值。方法对39例急危重心律失常患者的动态心电图数据,回顾性分析其Lorenz RR散点图特征。对其中窦性心律伴快速室性心动过速病例按窦性心律震荡正常与否分为正常组14例与异常组19例,比较两组之间的室速发生阵次、室性心搏发生率、平均心率的差异。结果缓慢心律失常伴心室停搏的Lorenz RR散点图表现为基础心律失常图形伴远端心室停搏散点;快速心律失常特征为基础心律失常图形伴45°线近端快速心动过速的散点集;窦性心律震荡正常组与异常组的室速阵次、室性心搏发生率及平均心率比较差异无统计学意义(P>0.05)。结论根据Lorenz RR散点图图形特征辅以逆向心电图分析技术可以快速发现危急值心电图,为患者赢得治疗时间;频发室性心律失常中的室速阵次、室性早搏发生率可能不是其高危猝死预警指标。
Objective To retrospectively analyze the dynamic electrocardiogram data of patients with acute and critical arrhythmia,analyze the characteristics of RR Lorenz plot,and explore the value of RR Lorenz plot in rapid diagnosis of acute and critical arrhythmia.Methods The dynamic electrocardiogram data of 39 patients with acute and critical arrhythmia were analyzed retrospectively.The patients with sinus rhythm and rapid ventricular tachycardia were divided into abnormal group(19 cases)and normal group(14 cases)according to whether sinus rhythm concussion was normal or not.The differences of ventricular tachycardia frequency,incidence of ventricular beat and average heart rate between the two groups were compared.Results The RR Lorenz plot of slow arrhythmia with ventricular arrest was based on arrhythmia pattern with distal ventricular arrest scatter plot;rapid arrhythmia was based on arrhythmia pattern with 45 degree line near-end rapid tachycardia scatter plot;ventricular tachycardia frequency,incidence of ventricular beats in abnormal sinus rhythm concussion group and normal group;There was no significant difference in average heart rate.Conclusions According to RR Lorenz plot and reverse electrocardiogram analysis technology,critical value electrocardiogram can be quickly found and treatment time can be saved for patients.Ventricular tachycardia and premature ventricular beats may not be independent early warning indicators of high-risk sudden death in frequent ventricular arrhythmia.
