摘要
目的研究通窍醒脑针法联合药物疗法对阿尔兹海默症患者的认知及吞咽功能的影响。方法选取医院2018年6月-2019年6月收治的100例阿尔兹海默症患者纳入研究对象,以随机数表法分为A、B、C、D四个组,每组25例。A组为不愿意治疗或无力承担治疗费用患者,给予安慰剂治疗,B组行通窍醒脑针法治疗,C组行盐酸多奈哌齐与盐酸美金刚联合治疗,D组行通窍醒脑针法及盐酸多奈哌齐、盐酸美金刚联合治疗。比较4组临床疗效,比较治疗前后吞咽功能障碍情况及简易智力状态检查量表(MMSE)、日常生活能力(ADL)评分,比较患者治疗期间不良反应发生情况。结果4组患者治疗后总有效率D组>B组与C组>A组,差异有统计学意义(P<0.05)。四组患者治疗前吞咽功能障碍分级对比无显著差异(P>0.05);4组患者治疗后吞咽功能障碍分级比较,差异存在显著统计学意义,吞咽功能正常患者占比D组>B、C组,差异有统计学意义(P<0.05)。4组患者治疗前MMSE、ADL评分对比差异无显著统计学意义(P>0.05);4组患者治疗1、4疗程后MMSE及ADL评分对比,D组>B、C组>A组,差异有统计学意义(P<0.05)。B、C、D三组患者治疗后不良反应总发生率对比差异无显著统计学意义(16.00%VS 24.00%VS 32.00%,P>0.05)。结论通窍醒脑针法及盐酸多奈哌齐、盐酸美金刚联合治疗阿尔兹海默症能够提高临床治疗效果,更好地改善患者认知功能,提高日常生活能力,且并不增加患者治疗期间的不良反应总发生率。
Objective to study the effect of acupuncture method of activating the body and activating the brain combined with drug therapy on cognition and swallowing function of patients with Alzheimer’s disease.Methods 100 patients with Alzheimer’s disease in our hospital from June 2018 to June 2019 were selected and randomly divided into four groups:A,B,C and D,with 25 cases in each group.Group A was given placebo treatment,group B was treated with acupuncture method of activating the body and activating the brain,group C was treated with combined with memantine hydrochloride,and group D was treated with acupuncture method of activating the body and activating the brain,donepezil hydrochloride and memantine hydrochloride.The clinical efficacy,swallowing dysfunction,MMSE and ADL scores before and after treatment were compared among the four groups,and the incidence of adverse reactions during the treatment was compared.Results after treatment,the total effective rate of group D was higher than that of group B and group C,and the difference was statistically significant(P<0.05).There was no significant difference in the classification of swallowing dysfunction among the four groups before treatment(P>0.05);there was significant difference in the classification of swallowing dysfunction after treatment among the four groups,and the proportion of patients with normal swallowing function in group D was higher than that in group B and group C(P<0.05).There was no significant difference in MMSE and ADL scores between the four groups before treatment(P>0.05);after 1 and 4 courses of treatment,MMSE and ADL scores of 4 groups were compared,and the difference was statistically significant(P<0.05).B.There was no significant difference in the total incidence of adverse reactions between group C and group D(16.00%vs 24.00%vs 32.00%,P>0.05).Conclusion acupuncture method of activating the body and activating the brain,combined with donepezil hydrochloride and memantine hydrochloride in the treatment of Alzheimer’s disease can improve the clinical treatment effect,better improve the cognitive function of patients,improve the ability of daily living,and do not increase the total incidence of adverse reactions during treatment.
