摘要
目的评价基于老年综合评估(CGA)的急性期快速恢复(ACE)病房个体化治疗对老年衰弱患者急性期恢复的效果。方法选取郑州大学附属郑州中心医院老年医学科2018年6月~2018年12月新入院的75岁以上急性期老年衰弱患者,根据患者意愿分为对照组与ACE组,对照组入住传统老年专科病房,采取传统医疗护理方案,ACE组入住ACE病房,采用基于CGA的个体化医疗护理方案。比较两组平均住院时间及90 d后的握力(HS)、计时起立行走测试(TUG)时间、6 min步行实验(6-MWT)距离的改善程度。结果ACE组平均住院日明显少于对照组(P<0.05);90 d后,与对照组相比,ACE组HS增加幅度、TUG时间缩短幅度、6-MWT距离增加幅度更加明显(P<0.05)。结论基于CGA的ACE病房个体化治疗能够明显提高老年衰弱患者的照护质量,缩短患者平均住院时间,改善老年衰弱患者的结局。
Objective To evaluate the effect of individualized treatment in acute care for the elderly(ACE)ward based on comprehensive geriatric assessment(CGA)on acute recovery of frail older patients.Methods The frail older patients(>75 years old)in acute phase from June 2018 to December 2018 were enrolled in Zhengzhou Central Hospital Affiliated to Zhengzhou University.The patients were divided into the control group and ACE group according to their wishes.The control group was admitted to the traditional geriatric ward and received the traditional medical care scheme.The ACE group was admitted to the ACE ward and received the individualized medical care plan based on CGA.The hospitalization days,handgrip strength(HS),timed up-and-go(TUG),6-minute walking test(6-MWT)in two groups were documented.Results The average hospitalization day in ACE group was significantly shorter than that in the control group(P<0.05).At 90 days after care,compared with the control group,the increase of HS,the decrease of TUG and the increase of 6-MWT distance in ACE group were more significant(P<0.05).Conclusion The individualized treatment based on CAG in ACE ward might significantly improve the quality of care,shorten the length of stay and improve the outcomes of frail older patients.
