髂骨内固定支架治疗骨盆后环损伤的疗效观察 被引量:2

摘要 目的探讨应用髂骨内固定支架治疗骨盆后环损伤的疗效。方法选取2011年1月至2018年12月南充市中心医院采用髂骨内固定支架治疗的87例骨盆后环损伤患者为研究对象。复位质量影像学评价采用Matta标准,临床功能评价采用Majeed功能评分。术后8~12个月如果骨折愈合,取出内固定物。结果76例患者获得4~24个月随访(平均12.1个月)。4例患者术后切口感染经清创、抗感染后治愈。至最后1次随访,有2例患者出现>5 mm的骨折再移位(分别是6、9 mm)。没有内固定物引起的神经血管损伤。骨折复位质量根据Matta标准评定:优59例,良15例,可2例。Majeed功能评分结果为优49例,良16例,可8例,差3例,优良率85.5%。结论髂骨内固定支架相对其他内固定物是治疗骨盆后环损伤的一种理想选择,具有操作简单,创伤小,出血少,软组织、血管、神经损伤并发症少等优点。 Objective To explore the efficacy of transiliac interal fixator(TIFI)for dorsal pelvic ring injuries.Methods A total of 87 patients with dorsal pelvic ring injuries treated with TIFI in Nanchong Central Hospital from Jan.2011 to Dec.2018 were selected as the study subjects.Matta criterion was used for imaging evaluation of reduction quality,and Majeed functional scoring was used for clinical function evaluation.The internal fixation was removed if the fracture was healed from 8 to 12 months after surgery.Results 76 patients were followed up for 4 to 24 months(mean 12.1 months).4 cases of postoperative wound infection were cured after debridement and anti-infection.At the last follow-up,there were 2 patients with fracture redisplacement greater than 5 mm(6 mm and 9 mm respectively).No neurovascular lesion due to internal fixation occurred.According to Matta criterion for fracture reduction,the results were excellent in 59 cases,good in 15 cases and fair in 2 cases.According to Majeed functional scoring,the results were excellent in 49 cases,good in 16,fair in 8,and poor in 3,with an excellent-good rate 85.5%.Conciusion With the advantages of simple operation,less trauma,less bleeding,fewer complications,and fewer risks of iatrogenic soft tissue and neurovascular damage,TIFI is an ideal alternative to other fixation devices for dorsal pelvic ring injuries.
