摘要
目的了解驻马店市艾滋病(AIDS)疫情特征,为制定防治策略提供科学依据。方法对1995~2019年驻马店市艾滋病防治基本信息系统中累计报告的AIDS相关数据进行描述性流行病学分析。结果1995~2019年驻马店市累计发现并报告人类免疫缺陷病毒感染者和病人(HIV/AIDS)17810例,现存活11491例。疫情呈现迅速上升、快速下降、趋于平稳3个时期;共报告死亡病例6319例,2005年达到高峰,2005~2017年报告病死率呈现逐年下降态势(P<0.01),但近3年病死率有上升趋势(P<0.05);病例主要集中在上蔡县,9135例(52.29%);年龄以30~39岁病例最多,6041例(33.92%),15~19和60岁以上年龄组男女性别比分别是3.65∶1和2.11∶1;感染途径中累计病例以经血传播为主,11466例(64.38%),其次是异性传播,4814例(27.03%),经血途径和母婴传播病例占当年报告病例总数的构成比呈现逐年下降趋势(P<0.01),经异性和同性性传播病例占当年报告病例总数的构成比呈逐年上升趋势(P<0.01)。结论性传播成为驻马店市新报告HIV/AIDS病例的主要传播途径,同性性传播增长趋势明显。
Objective To understand epidemiological characteristics of AIDS in Zhumadian city,so as to provide scientific evidence for developing strategies of prevention and control.Methods Descriptive epidemiological analysis was used on accumulated AIDS data from Basic Information System for AIDS Prevention and Control in Zhumadian city during 1995-2019.Results A total of 17810 cases of human immunodeficiency virus(HIV)infection and patients(AIDS)were detected and reported during 1995-2019,11491 cases of whom were alive.A total of 3 periods of epidemic tendency was observed,which had shifted from rapid rise to rapid decline and to stabilization as time went on.A total of 6319 cases of death were reported,which reaching a peak in 2005.It showed a decreasing trend of fatality rate year by year from 2005 to 2017(P<0.01),however it has been increasing in the last 3 years(P<0.05).Cases concentrated in Shangcai county(9135 cases,52.29%).People aged 30-39 years old had the largest number of cases(6041 cases,33.92%).The male to female ratio of cases aged 15-19 years and over 60 years was 3.65:1 and 2.11:1,respectively.Infection route of cumulative cases was given priority to blood transmission,accounting for 64.38%,followed by heterosexual transmission,accounting for 27.03%.It showed a decline trend of the proportion of blood ways and mother-to-child transmission year by year(P<0.01),and that of heterosexual and homosexual transmission rising year by year(P<0.01).Conclusion Sexual transmission has become the main transmission route for new reported HIV/AIDS cases in Zhumadian city,with an obvious growth trend of homosexual transmission.
