摘要
目的探讨替罗非班在急性心肌梗死急诊冠状动脉介入治疗中的应用价值。方法选择2018年1月至2019年1月本溪市中心医院102例急性心肌梗死患者,采用数字平均分配的方式,分为2个小组,常规组患者在实施冠状动脉造影前注射替罗非班,研究组患者在实施冠状动脉造影后注射替罗非班。结果两组患者在TIMI血流分级方面进行比较,研究组患者优于常规组(P<0.05)。研究组患者二磷酸腺苷、花生四烯酸、胶原的血小板聚集率低于常规组(P<0.05)。研究组患者左心室射血分数、左心室舒张末期内径、左心室收缩末期内径指标优于常规组(P<0.05)。研究组患者出现心绞痛、心功能不全、再次梗死以及心源性死亡的总发生率为3.92%,低于常规组17.64%(P<0.05)。结论对急性心肌梗死患者实施急诊冠状动脉造影后应用替罗非班效果理想,可调节心肌再灌注,控制血小板聚集,提高心室功能。
Objective To investigate the application value of Tirofiban in emergency coronary intervention therapy for acute myocardial infarction.Methods From January 2018 to January 2019,102 patients with acute myocardial infarction were selected from benxi central hospital,patients in the conventional group were injected with Tirofiban before coronary angiography,and those in the study group were injected with Tirofiban after coronary angiography.Results TIMI blood flow grading was compared between the two groups,and the study group was superior to the conventional group(P<0.05).The platelet aggregation rate of adenosine,arachidonic acid and collagen in the study group was lower than that in the conventional group(P<0.05).The indexes of left ventricular ejection fraction,left ventricular end-diastolic diameter and left ventricular end-systolic diameter in the study group were better than those in the conventional group(P<0.05).The incidence of angina,cardiac insufficiency,secondary infarction and cardiac death in the study group was 3.92%,lower than that in the conventional group(17.64%)(P<0.05).Conclusion Tirofiban is effective in acute myocardial infarction patients after emergency coronary angiography.It can regulate myocardial reperfusion,control platelet aggregation,and improve ventricular function.
