动态增强CT扫描与MRI诊断肝细胞癌的价值及影像学特征 被引量:3

Value and Imaging Features of Dynamic Contrast-enhanced CT and MRI in the Diagnosis of Hepatocellular Carcinoma

摘要 目的分析动态增强CT扫描与MRI诊断肝细胞癌的价值及影像学特征。方法选取2017年1月至2018年12月辽阳市中心医院收治的133例肝脏病变患者,先后行肝脏动态增强CT扫描与肝脏MRI检查,以病理检查结果为金标准,比较两种不同检查方法的诊断价值及影像学特征。结果病理检查结果示,肝细胞癌阳性75例,阳性率为56.39%;阴性58例,阴性率为43.61%。肝脏MRI检查对病灶诊断的灵敏度、特异度分别为94.67%(71/75)、77.59%(45/58),肝脏动态增强CT扫描分别为86.67%(65/75)、44.83%(10/58),差异均有统计学意义(均P<0.05)。动态增强CT扫描共检出肝细胞癌病灶83个,其中小肝癌病灶25个;MRI检查共检出肝细胞癌病灶80个,其中小肝癌病灶18个。肝细胞癌病灶主要表现出病灶动脉期迅速强化、门静脉期强化减退、平衡期与周围正常肝实质相比呈低密度改变的"快进快出"特点。结论应用MRI检查诊断肝细胞癌具有一定优势,临床应加强MRI在肝细胞癌检出、定性中的运用,其可为肝细胞癌的早期诊断和治疗提供准确的影像学依据。 Objective To analyze the value and imaging features of dynamic enhanced CT scan and MRI in the diagnosis of hepatocellular carcinoma.Methods From January 2017 to December 2018,133 patients with liver disease admitted to Liaoyang Central Hospital,the liver dynamic enhanced CT scan and liver MRI examination were performed successively,and the diagnostic value and imaging characteristics of the two different examination methods were compared using the pathological examination result as the gold standard.Results Pathological examination showed that 75 cases of hepatocellular carcinoma were positive with a positive rate of 56.39%;58 cases were negative with a negative rate of 43.61%.The sensitivity and specificity of liver MRI examination for diagnosis of lesions were 94.67%(71/75),77.59%(45/58),and the dynamic enhanced CT scans of liver were 86.67%(65/75),44.83%(10/58),the differences were statistically significant(all P<0.05).A total of 83 hepatocellular carcinoma lesions were diagnosed by dynamic enhanced CT scanning,including 25 small hepatocellular carcinoma lesions;80 hepatocellular carcinoma lesions were diagnosed by MRI,including 18 small hepatocellular carcinoma lesions.Hepatocellular carcinoma lesions mainly show the characteristics of rapid enhancement in the arterial phase of the lesion,reduction in enhancement in the portal vein phase,and a low density change in the equilibrium phase compared with the surrounding normal liver parenchyma.Conclusion Application of MRI in the diagnosis of hepatocellular carcinoma has certain advantages.Clinical application of MRI in the detection and characterization of hepatocellular carcinoma should be strengthened in order to provide accurate imaging basis for the early diagnosis and treatment of hepatocellular carcinoma.
