盐酸戊乙奎醚预处理对体外循环相关肺损伤大鼠肺表面活性因子的影响

摘要 目的探讨盐酸戊乙奎醚预处理对体外循环术后肺损伤大鼠肺表面活性因子的影响。方法选取健康成年SD大鼠30只,体重350~400g,将大鼠随机分为对照组(Control组)、体外循环组(CPB组)、盐酸戊乙奎醚预处理组(PHC组),每组10只。Control组仅进行麻醉、穿刺置管及机械通气,CPB组进行常规体外循环转流2h,PHC组于体外循环(CPB)转机前30min给予2ml/kg盐酸戊乙奎醚,Control组、CPB组给予等量生理盐水。于体外循环前30min(T0)、体外循环术后1h(T1)、4h(T2)、8h(T3)时间点取动脉血行血气分析,计算氧合指数(OI)和呼吸指数(RI),并用ELISA法检测血液中SP-A、SP-D、IL-6、IL-10水平,于T3时间点取血后处死大鼠,取部分肺组织用免疫组织化学染色法测定SP-B、SP-C的表达水平。结果与Control组相比,CPB组与PHC组均有不同程度的呼吸功能受损,血清学及蛋白表达异常。PHC组通气指标OI与RI水平均高于CPB组,在T1~T3时间点IL-6水平低于CPB组,IL-10水平高于CPB组;在T2、T3时间点SP-A、SP-D低于CPB组;并且在肺组织中SP-B、SP-C蛋白表达水平高于CPB组。结论PHC预处理能够通过抑制炎性因子释放减轻肺损伤,其机制与抑制肺泡表面活性蛋白A、D转移入血,保护肺泡表面活性蛋白B、C活性,维持肺泡结构与改善肺泡张力有关。 Objective To investigate the effect of pretreatment with penehyclidine hydrochloride on pulmonary surfactant in rats with cardiopulmonary bypass related lung injury.Methods 30 healthy SD rats weighted 350~400 g were randomly divided into 3 groups(n=10),control group received anesthesia and mechanical ventilation,cardiopulmonary bypass group(CPB group)received extracorporeal circulation for 2 hours,pretreatment penehyclidine hydrochloride group(PHC group)received 2 ml/kg penehyclidine hydrochloride 30 minutes before cardiopulmonary bypass.Control group and CPB group received an equal amount of normal saline.Arterial blood was taken for blood gas analysis and oxygenation index and respiratory index were calculated.SP-A,SP-D,IL-6,IL-10 level were detected by ELISA for 4 times,including the time of 30 minutes before cardiopulmonary bypass(T0),1 hour(T1),4 hours(T2)and 8 hours(T3)after cardiopulmonary bypass.The rats were euthanized at the time of T3 to detect the expression of SP-B,SP-C by immunohistochemical staining in lung tissue of the rats.Results Compared with the control group,CPB group and PHC group had different degrees of respiratory function impairment and the abnormalities of serological expression and protein expression.OI and RI in PHC group were higher than those in CPB group.At T1~T3 time point,IL-6 level in PHC group was lower,and IL-10 level was higher than that in CPB group.At T2 and T3 time point,SP-A and SP-D levels in PHC group were lower than those in CPB group.The expression of SP-B and SP-C in lung in PHC group were higher than those in CPB group.Conclusion Pretreatment with penehyclidine hydrochloride can reduce lung injury by inhibiting the release of inflammatory factors.The mechanism is related to inhibiting the transfer of alveolar surfactant protein A and D into blood,protecting the activity of surfactant protein B and C,maintaining alveolar structure and improving alveolar tension.
