不同孕周产前超声对胎儿畸形的筛查意义 被引量:2

A survey of the significance of prenatal ultrasound in screening fetal malformations in different gestational weeks

摘要 目的探讨不同孕周产前超声对胎儿畸形的筛查意义。方法将2017年8月~2019年8月在我院建档并按时产检,均进行孕早期、中期以及晚期超声检查后进行分娩或者引产的2625例孕妇的相关资料进行回顾性分析。结果不同孕期共筛查出胎儿畸形78例(86项畸形)。经引产及娩出后证实的胎儿先天畸形共89项,检出率为96.63%,未检出的3项为多指1例,单纯性腭裂1例,室间隔缺损1例。孕中期发现的胎儿畸形最多,为57例(66.23%);不同系统而言,则中枢神经系统畸形最为多见,为24例(27.91%),其次为胸腔腹壁、心血管系统及颈面部。与孕中期比较,孕早期、晚期的畸形检出率均降低明显,差异有统计学意义(χ2=21.568,P<0.001)。与孕中期比较,孕早期、晚期的中枢神经系统、心血管系统、颈面部的检出率均降低明显,差异有统计学意义(χ2=12.892、11.763、9.262;P<0.001)。结论孕早期可筛查部分严重的胎儿畸形,孕中期可检出大多数先天畸形,孕晚期对于迟发型畸形有诊断意义,掌握不同孕周的超声筛查重点,可提高胎儿畸形的检出率,实现优生优育。 Objective To explore the significance of prenatal ultrasound in screening fetal malformations in different gestational weeks.Methods The data of 2625 pregnant women filed in our hospital from August 2017 to August 2019 who underwent prenatal ultrasound examination on time in early,middle and late pregnancy and gave birth or got induced labor were retrospectively analyzed.Results A total of 78 cases(86 malformations)were screened out over different pregnancy periods.There were 89 congenital malformations confirmed after induction of labor and delivery,with a detection rate of 96.63%.The undetected 3 malformations were multiple fingers in 1 case,simple cleft palate in 1 case and ventricular septal defect in 1 case.The most fetal abnormalities were found in the second trimester of pregnancy,with 57 cases(66.23%);For different systems,central nervous system malformation was the most common,with 24 cases(27.91%),followed by thoracic abdominal wall,cardiovascular system and neck or face.Compared with the second trimester of pregnancy,the detection rate of malformation in the first trimester and the third trimester of pregnancy decreased significantly,with statistically significant difference(χ2=21.568,P<0.001).Compared with that of the second trimester of pregnancy,the detection rates of central nervous system,cardiovascular system,neck or face in the first trimester and the third trimester of pregnancy were significantly lower,and the difference was statistically significant(χ2=12.892,11.763,9.262;P<0.001).Conclusion Certain severe fetal malformations can be screened in the first trimester of pregnancy,and most congenital malformations can be detected in the second trimester of pregnancy.The third trimester of pregnancy is of diagnostic significance for lateonset malformations.Mastering the key points of ultrasound screening in different gestational weeks can improve the detection rate of fetal malformations and realize better birth and care.
