持续质量改进在静脉用药调配中心抗肿瘤药物配制质量中的应用 被引量:32

摘要 目的探讨持续质量改进在静脉用药调配中心(PIVAS)抗肿瘤药物配制质量中的应用价值。方法选取持续质量改进管理模式于2018年7月至2019年6月在驻马店市中心医院PIVAS应用于抗肿瘤药物配制质量中的各个环节,比较实施前(2018年1月~2018年6月)、实施初期(2018年7月~2018年12月)以及实施稳定期(2019年1月~2019年6月)的相关工作指标(审方时间、配置时间、排药时间、周转天数、失误和发现不合格医嘱)、肿瘤用药护理人员的满意度。结果实施前抗肿瘤用药调配数12688次,实施初期13469次,实施稳定期13885次。实施初期、实施稳定期的审方时间、配置时间、排药时间和周转天数与实施前比较明显较少,且实施稳定期与实施初期比较明显较少,差异均有统计学意义(均P<0.05)。实施初期和实施稳定期的失误率分别为0.2%和0.1%,与实施前的0.46%比较明显较低,且实施稳定期与实施前比较明显较低,差异均有统计学意义(均P<0.05)。实施初期、实施稳定期的发现不合格医嘱率分别为0.1%和0.0%,与实施前的0.2%比较明显较低,且实施稳定期与实施前比较明显较低,差异均有统计学意义(均P<0.05)。共对114名护理人员进行调查。实施初期、实施稳定期的肿瘤用药护理人员满意度优于实施前,差异均有统计学意义(均P<0.01),实施稳定期的肿瘤用药护理人员满意度优于实施初期,差异有统计学意义(P<0.01)。实施初期和实施稳定期的总满意度分别为94.7%和85.1%,高于实施前的73.7%,且实施稳定期的总满意度高于实施初期,差异均有统计学意义(均P<0.05)。结论将持续质量改进管理模式应用于PIVAS,提高了抗肿瘤药物配制效率,减少了失误率和不合格医嘱率,有效保障了抗肿瘤药物的用药安全。 Objective To explore the significance of continuous quality improvement in antitumor drugs formulated by intravenous drug allocation center(PIVAS).Methods The continuous quality improvement management model was implemented in all aspects of quality improvement for antitumor drug formulation in PIVAS at Zhumadian Central Hospital between July 2018 and July 2019.Comparisons were made on relevant work indicators(prescription-checking time,formulating time,dispensing time,days of turnover,errors and discovery of unqualified orders)before implementation(from January 2018 to June2018)and in the early stages of implementation(from 2018 July to December 2018)and during the stabilization period(January 2019 to June 2019)as well as nurse satisfaction with the use of antitumor drug.Results Before implementation,12688 prescriptions were dispensed compared with 13469 of in the initial stage and 13885 during the stabilization period.Prescription-checking time,formulating time,dispensing time and days of turnover were significantly less in the early stage and during the stabilization period(all P<0.05).Error rate was 0.2%and 0.1%,respectively in the early stage and during the stabilization period which was significant lower than 0.46%of before implementation(all P<0.05).0.1%and 0.0%orders was found unqualified,respectively in the early stage and during the stabilization period which was significant lower than 0.2 of before implementation(all P<0.05).A total of 114 nurses were investigated.Nurse satisfaction with the use of antitumor drug was better in the early stage and during the stabilization period than before implementation(all P<0.05).The overall satisfaction rate was 94.7%and 85.1%,respectively in the early stage and during the stabilization period which was higher than 73.7%before implementation(all P<0.05).Conclusion Continuous quality improvement in antitumor drugs formulated by PIVAS improves the efficiency of anti-tumor drug preparation,reduces the error rate and the rate of unqualified orders,and effectively guarantees the safety of anti-tumor drug administration.
