摘要
目的探讨加速康复外科(ERAS)在脑肿瘤手术麻醉中的应用。方法选取2015年6月至2018年6月间陕西省商洛市中心医院收治的60例行脑肿瘤切除患者,根据治疗方法不同进行分组。采用常规围术期麻醉管理方案的30例患者纳入对照组,采用ERAS指导下围术期麻醉管理的30例患者纳入观察组。对两组患者的预后指标进行统计学比较。结果两组患者手术时间、麻醉时间、再次手术率、再次住院率及术后并发症发生率比较,差异无统计学意义(P>0.05)。两组患者麻醉前及麻醉复苏后体温比较,差异无统计学意义(P>0.05),观察组麻醉后术中体温均高于对照组,差异均有统计学差异(均P<0.05)。观察组术后24h及48h镇痛效果均优于对照组,差异均有统计学意义(均P<0.05)。结论应用ERAS麻醉管理模式能够改善脑肿瘤手术患者围术期镇痛效果,缩短患者麻醉复苏、术后进食、下床和拔除尿管时间,缩短住院时间,促进患者康复。
Objective To investigate the application of enhanced recovery after surgery(ERAS)in anesthesia in brain tumor surgery.Methods Sixty patients undergoing intracranial tumorectomy at Shangluo Central Hospital from June 2015 to June 2018 were selected.They were randomly divided into an observation group and a control group according to the treatment they received with 30 patients in each group.Patients in the observation group were given perioperative anesthesia management guided by ERAS and patients in the control group were given conventional perioperative anesthesia management.The prognostic indicators were compared between the two groups.Results There was no significant difference in operation time,anesthesia time,reoperation rate,readmission rate and incidence of complications between the two groups(P>0.05).The difference in body temperature in the two groups was not significant before anesthesia and after anesthetic recovery(P>0.05).Intraoperative body temperature was higher in the observation group than in the control group after anesthesia(P<0.05).Analgesic effect was better in the observation group than in the control group at 24 h and 48 h after the operation(all P<0.05).Conclusion ERAS-based anesthesia management can improve the perioperative analgesic effect after intracranial tumorectomy,shorten anesthetic recovery time,time to postoperative feeding,time to out-of-bed activity,time to urinary catheter removal,and length of hospital stay,and promote patient recovery.
