摘要
目的研究反流性食管炎患者胃镜下表现与中医证型及舌象的相关性。方法回顾性分析2018年3月—2019年5月收治的86例反流性食管炎患者流行病学信息,对所有患者的中医证型、舌象和其胃镜下表现进行观察和分析。结果不同证型的胃镜分级存在明显差异(P<0.05);不同证型的黏膜表现未见明显差异(P>0.05);不同舌色、舌苔的食管黏膜表现无明显差异(P>0.05),但食管黏膜粗糙不平多见于淡红舌,其他损伤类型多见于红舌;食管黏膜充血、水肿、渗出物增多、糜烂多见于薄黄苔,黏膜粗糙不平多见于白腻苔。结论反流性食管炎患者中医证型和胃镜下食管黏膜分级有关,舌象可反映食管黏膜情况,临床检查将现代医学诊断和中医辨证论结合,可更加准确诊断反流性食管炎患者疾病情况。
Objective To study the correlation between the gastroscopic manifestations of patients with reflux esophagitis and TCM syndromes and tongue images.Methods The epidemiological information of 86 patients with reflux esophagitis admitted in our hospital from March 2018 to May 2019 was retrospectively analyzed,and the TCM syndrome types,tongue image and gastroscopic performance of all patients were observed and analyzed.Results There were significant differences in gastroscopic classification of different syndrome types(P<0.05).There was no significant difference in mucosal manifestations of different syndrome types(P>0.05).There was no significant difference in esophageal mucosa manifestations of different tongue colors and tongue coatings(P>0.05).However,rough esophageal mucosa is more common in the red tongue,and other types of damage are more common in the red tongue;esophageal mucosa is congested,edema,exudate increased,erosion is more common in thin yellow moss,and rough mucosa is more common in white greasy moss.Conclusion TCM syndromes of patients with reflux esophagitis are related to the classification of esophageal mucosa under gastroscopy.Tongue images can reflect the situation of esophageal mucosa.Clinical examination can combine modern medical diagnosis with TCM syndrome differentiation to more accurately diagnose the disease of reflux esophagitis patients.
