摘要
目的分析肾移植受者肺孢子菌肺炎的流行病学特点。方法回顾性分析中国科技大学附属第一医院(安徽省立医院)2016年6月至2019年6月收治的371例肾移植受者的临床资料。监测肾移植受者肺孢子菌肺炎发生情况,分析感染特点。结果随访期间共确诊肺孢子菌肺炎受者60例,感染率为16.17%。2016年6月至2019年6月,该院肾移植受者肺孢子菌肺炎发生率持续下降(P<0.05)。60例肺孢子菌肺炎感染者中,男46例,女14例,年龄(36.92±5.15)岁,不同性别肾移植受者肺孢子菌肺炎发生率、年龄和病程比较差异均无统计学意义(均P>0.05)。肾移植受者肺孢子菌肺炎集中发生于肾移植术后6个月内,占比为93.33%,感染者临床表现以发热、发绀、呼吸困难为主,死亡率为5.00%。结论肾移植受者肺孢子菌肺炎发生率较高,集中于术后6个月内,死亡率较高,应强调肾移植受者术后肺孢子菌肺炎的早期预防、诊断与治疗。
Objective To investigate the epidemiological characteristics of pneumocystis carinii pneumonia in renal transplant recipients.Method The clinical data of 371 renal transplant recipients admitted to the First Affiliated Hospital of University of Science and Technology of China(Anhui Provincial Hospital)from June 2016 to June 2019 were retrospectively analyzed.The incidence of pneumocystis carinii pneumonia in renal transplant recipients and the characteristics of infection were monitored and analyzed.Result A total of 60 patients with pneumocystis carinii pneumonia were diagnosed during the follow-up period,the infection rate was 16.17%.From June 2016 to June 2019,the infection rate of pneumocystis carinii pneumonia in renal transplant recipients in the hospital continued to decline(P<0.05).There were 46 males and 14 females with an average age of(36.92±5.15)years old.There were no significant differences in the incidence of pneumocystis carinii pneumonia,age and course of disease between renal transplant recipients of different sexes(all P>0.05).The infection rate of pneumocystis carinii pneumonia was 93.33%in 6 months after operation.The main clinical manifestations were fever,cyanosis and dyspnea.The mortality rate was 5.00%.Conclusion The incidence of pneumocystis carinii pneumonia in renal transplant recipients is high,and the incidence of pneumocystis carinii pneumonia infection is high within 6 months after operation,and the mortality is high.The early prevention,diagnosis and treatment of pneumocystis carinii pneumonia should be emphasized.
