椎体单侧骨水泥注入PVP术治疗骨质疏松性椎体压缩骨折的临床分析 被引量:1

摘要 目的分析椎体单侧骨水泥注入经皮椎体成形术(PVP术)治疗骨质疏松性椎体压缩骨折的临床效果。方法71例骨质疏松性椎体压缩性骨折患者,均采用椎体单侧骨水泥注入PVP术治疗。比较患者术前和术后视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)评分、Cobb角,观察记录患者的并发症发生情况。结果患者术前VAS评分为(7.63±3.11)分、ODI评分为(63.19±7.84)分,术后72 hVAS评分为(3.75±2.611)分、ODI评分为(28.94±10.12)分,术后6个月的VAS评分为(2.12±1.03)分、ODI评分为(20.05±6.54)分;术前、术后72 h以及术后6个月VAS、ODI评分比较,差异具有统计学意义(P<0.05)。术后Cobb角(15.86±2.04)°小于术前的(19.47±1.20)°,差异具有统计学意义(P<0.05)。术后发生骨水泥椎体间隙渗漏3例,并发症发生率为4.23%(3/71)。结论椎体单侧骨水泥注入PVP术治疗骨质疏松性椎体压缩骨折可帮助患者减轻痛苦,术后ODI评分改善显著,临床效果较佳,是帮助患者恢复伤椎功能、改善生活质量的有效术式,值得推荐应用。 Objective To analyze the clinical effect of vertebral unilateral bone cement injection percutaneous vertebroplasty(PVP)in the treatment of osteoporotic vertebral compression fractures.Methods A total of 71 patients with osteoporotic vertebral compression fractures all treated by vertebral unilateral bone cement injection PVP.The visual analogue scale(VAS)scores,Oswestry dysfunction index(ODI)scores,and Cobb angles before and after surgery were compared,and the occurrence of complications was observed and recorded.Results Before surgery,VAS score was(7.63±3.11)points and ODI score was(63.19±7.84)points,which were(3.75±2.611)and(28.94±10.12)points 72 h after surgery,and(2.12±1.03)and(20.05±6.54)points 6 months after surgery.There were statistically significant differences in VAS and ODI scores before surgery,72 h after surgery,and 6 months after surgery(P<0.05).Cobb angle(15.86±2.04)°after surgery was less than that before surgery(19.47±1.20)°,and the difference was statistically significant(P<0.05).There were 3 cases of bone cement intervertebral space leakage after surgery,and the complication rate was 4.23%(3/71).Conclusion Vertebral unilateral bone cement injection PVP for the treatment of osteoporotic vertebral compression fracture can help patients to relieve pain.The postoperative ODI score is improved significantly and the clinical effect is satisfactory.It is effective for helping patients to restore the function of injured vertebra and improve their quality of life.The procedure is worth recommending.
