摘要
目的:应用逐步回归法探索超声特征在甲状腺功能异常的诊断价值,筛选出重要的超声参数。方法:收集2017年3月至2019年3月初次就诊,未接受治疗的甲状腺功能异常患者136例,患者均进行甲状腺超声和血清学检查。以促甲状腺激素与抗过氧化物酶抗体进行分组,分别评价其超声特征的统计学意义。应用逐步回归法筛选出有意义的参数,计算模型效能。结果:甲状腺实质回声、血流分布、峰值流速及舒张末期流速中,甲状腺功能正常组与甲状腺功能减退组比较,差异具有统计学意义(P<0.05)。阻力指数中,甲状腺功能正常组与甲状腺功能减退组比较,差异具有统计学意义(P<0.05)。促甲状腺激素组中,逐步回归法筛选出实质回声为重要特征。抗过氧化物酶抗体组中,逐步回归法筛选出实质内血流丰富程度为重要特征。结论:超声特征中的甲状腺实质回声与实质内血流丰富程度对判断甲状腺功能具有较强实用价值。
Objective The stepwise regression method was used to explore the diagnostic value of ultrasound features in thyroid dysfunction,and to screen out important ultrasound parameters.Methods 136 untreated patients with thyroid dysfunction during their first visit from March 2017 to March 2019 were collected.All patients underwent thyroid ultrasound and serological examination.Thyroid stimulating hormone and antiperoxidase antibody were grouped into groups,and the statistical significance of their ultrasound characteristics was evaluated respectively.Use stepwise regression to filter out meaningful parameters and calculate model performance.Results In thyroid parenchymal echo,blood flow distribution,peak flow velocity and end-diastolic flow velocity,the difference between the normal thyroid function group and the hypothyroidism group was statistically significant(P<0.05).In the resistance index,the difference between the normal thyroid function group and the hypothyroidism group was statistically significant(P<0.05).In the thyroid-stimulating hormone group,the parenchymal echo was selected as an important feature by the stepwise regression method.In the antiperoxidase antibody group,the stepwise regression method was used to screen the richness of blood flow in the parenchyma as an important feature.Conclusions The echo of the thyroid parenchyma and the abundance of blood flow in the parenchyma in the ultrasound features have strong practical value in judging thyroid function.
