摘要
目的:通过了解上海市各级医院医生职业高原现状,以及影响医生职业高原的人口学因素,从而采取有效的措施防范医生职业高原现象。方法:采用职业生涯问卷对上海市3所三级医院,8所二级医院,10所基层社区医院的249名医生进行问卷调查,并通过多元logistic回归分析影响医生职业高原现象的人口学因素。结果:上海市医生总体高原均分为(3.17±0.27)分,层级高原得分为(3.37±0.56)分,内容高原得分为(3.11±0.46)分,中心化高原得分为(3.01±0.47)分,其中三级医院的总职业高原得分为(2.91±0.31),二级医院职业高原得分为(3.18±0.34),基层社区医院职业高原总得分为(3.39±0.24)。多元线性回归分析发现,影响医生职业高原主要因素包括年龄、医生近一年进修次数;此外,各医院在不同维度职业高原影响因素上,具有差异化表现。结论:各级医院医生均处于潜高原水平,其中基层医院职业高原得分最高,相关部门应根据各医院层级个性化制定医生职业发展方式,以提高各级医院医生工作绩效,工作积极性、主动性及创新性,促使其为患者提供优质的医疗服务。
Objective To understand the status of career plateau of doctors in Shanghai and its influencing factors,and put forward countermeasures.Methods 249 doctors from 3 tertiary hospitals,8 secondary hospitals and 10 grassroots community hospitals in Shanghai were surveyed with a career questionnaire.And the demographic factors influencing doctors’career plateau were analyzed by multivariate logistic regression.Results The overall average score of career plateau of doctors in Shanghai was 3.39±0.24,the grade score was 3.63±0.52,the content score was 3.44±0.42,and the centralized score was 3.10±0.41.The overall score of career plateau of doctors in tertiary hospital was 2.9±0.31,3.18±0.34 in secondary hospital,and 3.39±0.24 in grassroots community hospitals.Multiple linear regression analysis showed that the main factors to influence the career plateau of doctors were age and the times of doctors’training in the past year.In addition,the influencing factors in different dimensions had different performances in different hospitals.Conclusion Doctors in different hospitals are in latent plateaus,in which the doctors in grassroots hospitals had the highest scores of career plateaus.It is suggested for relevant departments to formulate doctors’career development plan to improve their performance,work enthusiasm,initiative and innovation.
