使用低黏度骨水泥和高黏度骨水泥实施经皮穿刺椎体成形术的临床疗效比较 被引量:8

Comparison of Clinical Efficacy of Percutaneous Vertebroplasty with Low Viscosity Bone Cement a High Viscosity Bone Cement

摘要 目的:比较使用低黏度骨水泥和高黏度骨水泥实施经皮穿刺椎体成形术(percutaneous vertebroplasty,PVP)治疗骨质疏松性胸腰椎椎体骨折(osteoporotic vertebral compression fracture,OVCF)的骨水泥渗漏风险情况及其疗效。方法:前瞻性观察高黏水泥组、高黏水泥低黏配置组和低黏水泥组三组使用不同黏度的骨水泥行PVP治疗骨质疏松骨折的病例。记录术前、术中、术后第2天的疼痛评分,留存所有术前、术中、术后的影像资料,观察骨水泥渗漏情况的发生特点,比较各组发生骨水泥渗漏的情况、骨水泥分布情况。结果:三组病例中骨水泥渗漏发生率差异无统计学意义。相对高黏水泥组,高黏水泥低黏配置组和低黏水泥组骨水泥填充率和扩散度更大,术中推注骨水泥时疼痛程度较轻,差异具有统计学意义。结论:在PVP治疗OVCF中,使用低黏度骨水泥和相对低黏期推注骨水泥,不增加骨水泥渗漏发生率,而骨水泥扩散度更大,骨水泥在椎体内的充填率更高,推注骨水泥时疼痛程度较轻。 Objective To compare the risk of bone cement leacage and efficacy of percutaneous vertebroplasty(PVP)using low viscosity bone cement and high viscosity bone cement in the treatment of osteoporotic vertebral compression fracture(OVCF).Methods Prospective observation was conducted on high viscosity cement group,high viscosity cement low viscosity configuration group and low viscosity cement group using different viscosity of bone cement for PVP treatment of OVCF cases.The pain scores of preoperative,intraoperative and day 2 after operation were recorded.All preoperative,intraoperative and postoperative imaging data were retained.The occurrence characteristics of bone cement leakage were observed,and the situation of bone cement leakage and bone cement distribution in each group were compared.Results There was no statistically significant difference in the incidence of bone cement leakage among the three groups.Compared with the high-viscosity cement group,the high-viscosity cement low-viscosity configuration group and the low-viscosity cement group had greater bone cement filling rate and diffusivity,and the pain was lighter during the intraoperative injection of bone cement.The difference was statistically significant.Conclusion In the treatment of OVCF with PVP,the use of low-viscosity bone cement and relatively low-viscosity injection of bone cement did not increase the incidence of bone cement leakage,and the diffusion rate of bone cement was greater,and the filling rate of bone cement in the vertebral body was higher.The pain was milder when pushing bone cement.
