摘要
目的研究糖尿病视网膜病变(diabetic retinopathy,DR)的中医证候类型与颈动脉超声检查结果的相关性。方法收集309例DR颈动脉超声资料,比较不同中医证候类型DR患者的颈动脉超声检查结果,并使用统计学方法分析获得的数据,总结证候类型与颈动脉超声之间的关系。结果DR中医证候类型的分布从高到低依次是气阴两虚证>肝肾亏虚证>阴阳两虚证,组间比较,P<0.05,差异明显,认为有统计学意义。中医证候类型的年龄统计情况从高到低依次是阴阳两虚证>肝肾亏虚证>气阴两虚证(P<0.05)。不同证候类型的颈动脉粥样硬化分级情况存在差异。不同证候类型颈动脉血流PSV、EDV从高到低均为气阴两虚>肝肾亏虚>阴阳两虚,RI从高到低依次是阴阳两虚>肝肾亏虚>气阴两虚(P<0.05)。结论DR各中医证候类型与颈动脉超声检查得出的有关颈动脉结构、功能改变的相关指标有一定的相关性,在一定程度上能为DR诊断和辨证的客观化提供参考。
Objective To study the correlation between TCM syndromes of diabetic retinopathy and the results of carotid artery ultrasound.Methods The data of 309 cases of DR patients were collected to compare the results of carotid artery ultrasound examination of DR patients with different TCM syndrome types,and the data obtained was analyzed by statistical methods to summarize the relationship between the syndrome types of DR patients and carotid artery ultrasound.Results The distribution of TCM syndromes of DR patients from high to low in order of syndrome was deficiency of Qi and Yin>syndrome of deficiency of liver>syndrome of deficiency of Yin and Yang,and the difference was significant between groups(P<0.05).The statistical data of age of each TCM syndrome type of DR patients from high to low was syndrome of deficiency of Yin and Yang>syndrome of deficiency of liver and kidney>syndrome of deficiency of Qi and Yin(P<0.05).There were differences in the classification of carotid atherosclerosis in DR patients with different syndromes.Different syndrome types of carotid artery blood flow PSV and EDV from high to low were syndrome of deficiency of Qi and Yin>syndrome of deficiency of liver>syndrome of deficiency of Yin and Yang,RI from high to low was syndrome of deficiency of Yin and Yan>syndrome of deficiency of liver and kidney>syndrome of deficiency of Qi and Yin(P<0.05).Conclusion There is a certain correlation between the types of TCM syndromes of DR and the related indexes of carotid artery structure and function changes obtained by carotid artery ultrasound examination,which can provide reference for the diagnosis and syndrome differentiation of DR to a certain extent.
