摘要
目的研究评估二维联合三维超声自由解剖成像技术(OmniView)在胎儿腭裂的诊断及分类上的应用价值。方法采集112例产前发现唇腭裂胎儿的唇腭部的三维容积数据,应用OmniView技术进行分析,比较单独二维超声和联合OmniView技术对腭裂诊断及分类的灵敏度。结果 (1)单独二维超声与联合OmniView技术在诊断唇裂合并原发腭裂、唇裂合并不完全腭裂、唇裂合并完全腭裂、单纯腭裂、黏膜下腭裂中的灵敏度分别为100%(19/19)与100%(19/19)、0%(0/4)与50%(2/4)、100%(61/61)与100%(61/61)、83.33%(5/6)与100%(6/6)、100%(1/1)与100%(1/1),差异均无统计学意义(P> 0.05)。(2)单独二维超声与联合OmniView技术在准确诊断腭裂分类上的灵敏度分别为93.48%、97.83%,联合OmniView技术后灵敏度有所提高。结论三维OmniView技术能够直观的显示胎儿腭部,帮助明确腭裂分类,有助于临床评估。
Objective To evaluate the value of two-dimensional combined three-dimensional ultrasonogra-phy Omni View in the diagnosis and classification of fetal cleft palate.MethodsThe three-dimensional volumedata of lip and palate of 112 fetuses with cleft lip and palate were collected and analyzed by Omni View technique.The diagnostic results were compared with those of single two-dimensional ultrasound. The sensitivity of cleft palatediagnosis and classification by two-dimensional ultrasound alone and combined with Omni View technique were com-pared.Results(1)The sensitivity of single two-dimensional ultrasound and combined Omni View in the diagnosisof cleft lip with primary cleft palate were 100%(19/19)and 100%(19/19), in cleft lip with incomplete cleftpalate were 0%(0/4)and 50%(2/4),in cleft lip with complete cleft palate were 100%(61/61)and 100%(61/61),in simple cleft palate were 83.33%(5/6)and 100%(6/6),and finally in submucosal cleft palate were 100%(1/1)and 100%(1/1),The difference was not statistically significant(P > 0.05).(2)The sensitivity of single two-dimensional ultrasound and combined Omni View in the accurate diagnosis of cleft palate was 93.48% and 97.83%respectively,and the sensitivity was improved after combined with Omni View.Conclusion3 D Omni View tech-nique can directly display the palate of fetus,facilitates to determine the classification of cleft palate,which ishelpful for clinical evaluation.
