摘要
目的探讨纤维支气管镜灌洗联合氨溴索肺泡灌洗治疗老年重症肺炎伴肺不张的疗效。方法选取我院2017年7月至2019年3月收治的95例老年重症肺炎伴肺不张患者,随机分为对照组(47例)和观察组(48例)。对照组采用纤维支气管镜灌洗治疗,观察组在对照组基础上采用氨溴索肺泡灌洗治疗,比较两组的临床疗效、炎性指标、通气时间、住院时间及肺不张恢复时间。结果观察组的总有效率为89.58%,高于对照组的70.21%(P <0.05)。治疗前,两组的PCT、 CRP、 WBC水平比较无统计学差异(P>0.05);治疗后,两组的PCT、 CRP、 WBC水平明显降低,且观察组的PCT、 CRP、 WBC水平低于对照组(P<0.05)。观察组的通气时间、肺不张恢复时间及住院时间均短于对照组(P <0.05)。结论纤维支气管镜灌洗联合氨溴索肺泡灌洗可有效减轻老年重症肺炎伴肺不张患者机体炎性反应,改善通气功能,缩短住院时间,提高疗效。
Objective To explore the efficacy of bronchofiberscope lavage combined with ambroxol alveolar lavage in the treatment of senile severe pneumonia complicated with atelectasis.Methods 95 elderly patients with severe pneumonia complicated with atelectasis in our hospital from July 2017 to March 2019 were selected and randomly divided into control group(47 cases)and observation group(48 cases).The control group was treated with bronchofiberscope lavage,and the observation group was treated with ambroxol alveolar lavage on this basis.The clinical efficacy,inflammatory indicators,ventilation time,hospitalization time and recovery time of atelectasis were compcared between the two groups.Results The total effective rate of the observation group was 89.58%,higher than 70.21%of the control group(P<0.05).After treatment,the PCT,CRP and WBC levels of the two groups decreased significantly,and the PCT,CRP and WBC levels of the observation group were lower than those of the control group(all P<0.05).The ventilation time,recovery time of atelectasis and hospitalization time in the observation group were shorter than those of the control group(P<0.05).Conclusions Bronchofiberscope lavage combined with ambroxol alveolar lavage can effectively reduce the body inflammatory response of elderly patients with severe pneumonia complicated with atelectasis,improve the ventilation function,shorten the hospitalization time and improve the efficacy.
