摘要
目的:总结Da Vinci Xi机器人胰十二指肠切除术的经验及应用体会。方法:回顾分析2019年8月至2019年12月5例接受Da Vinci Xi机器人胰十二指肠切除术患者的临床资料。结果:5例患者均在完全Da Vinci Xi机器人下完成手术,手术均为完全切除。手术时间240~390 min,中位手术时间270 min;术中出血量50~200 mL,中位术中出血量100 mL;术后住院10~18 d,中位术后住院时间13 d。术后1例患者出现胰瘘,经充分引流等保守治疗后痊愈,围手术期无死亡病例发生。术后病理结果为:胆管高级别上皮内瘤变1例、十二指肠绒毛管状腺瘤1例、胆总管下端腺癌2例、十二指肠腺癌1例。结论:在熟练完成腹腔镜胰十二指肠切除术及灵活应用机器人手术系统的基础上开展Da Vinci Xi机器人胰十二指肠切除术是安全、可行的,具有一定的临床价值及推广意义。
Objective:To summarize the experience and application of Da Vinci Xi robotic pancreatoduodenectomy.Methods:The clinical data of 5 patients who received Da Vinci Xi robotic pancreatoduodenectomy from Aug.2019 to Dec.2019 were retrospectively analyzed.Results:All of the 5 patients were successfully treated with Da Vinci Xi robot.The operative time was 240-390 min,and the median operative time was 270 min.The intraoperative blood loss was 50-200 mL,and the median intraoperative blood loss was 100 mL.The postoperative hospital stay was 10-18 d,and the median postoperative hospital stay was 13 d.One patient developed pancreatic fistula after operation,and recovered after conservative treatment such as sufficient drainage.There was no death in the perioperative period.Postoperative pathological results showed 1 case of high grade intraepithelial neoplasia of the bile duct,1 case of tubulovillous adenoma of the duodenum,2 cases of lower segment adenocarcinoma of common bile duct,and 1 case of duodenal adenocarcinoma.Conclusions:It is safe and feasible to carry out Da Vinci Xi robotic pancreatoduodenectomy on the basis of proficient operation of laparoscopic pancreatoduodenectomy and flexible use of robotic surgery system,which has certain clinical value and promotion significance.
