摘要
目的通过分析首次CD4+淋巴细胞检测结果及影响因素,了解深圳市新报告HIV病例的晚发现情况,同时为艾滋病精准干预措施的制定提供数据支持。方法选取2016年深圳市进行了首次CD4^+淋巴细胞计数检测的新报告HIV感染者作为研究对象,运用t检验、单因素方差分析比较不同特征人群的CD4^+淋巴细胞数均值,用Logistic回归进行病例晚发现的相关因素分析。结果共有2 419例感染者纳入本次研究,其中近1/4的病例首次CD4^+数量低于200个/μl。不同人群之间的CD4+细胞均值比较:男性、小于25岁、未婚、大专及以上学历、同性传播的CD4+细胞数值较高,医院及羁押场所人员较低,汉族和少数民族之间差异无统计学意义。结论年龄大、学历低、医院和羁押场所来源的报告病例,其晚发现的可能性高。应加强对文化程度低、年纪大人群的艾滋病防治宣传教育,提高检测咨询的可及性,以最大限度尽早发现HIV感染者。
Objective To analyze the results of the first CD4^+lymphocytes test and its influencing factors,learn about the late detection of newly reported human immunodeficiency virus(HIV)cases in Shenzhen,in order to provide data support for the formulation of precise HIV intervention measures.Methods The newly reported HIV cases that underwent the first CD4^+lymphocytes count test in Shenzhen in 2016 were selected as the study subjects.T-test and ANOVA were used to compare the mean CD4^+lymphocytes number of people with different characteristics.Logistic regression was performed to analyze the related factors of the cases of late detection.Results A total of 2419 patients were involved in this study,among them,nearly a quarter of the cases whose first CD4^+count were less than 200/μl.The mean value of CD4^+cells was higher in male,under 25 years old,unmarried,college degree or above,and homosexual transmission.The lower number of CD4+cells was found in the cases from hospitals and places of custody.There was no significant difference between ethnic groups of Han and the minority.Conclusion The late detection was more likely to be found in cases of older age,low educational background,and in hospital and detention places.More health education should be given to the less educated and older population to promote the prevention and treatment of AIDS,and the accessibility of test and counseling should be improved,so as to detect the HIV cases as early as possible.
