摘要
目的了解天津市艾滋病病毒(HIV)阳性青年男男性行为者(MSM)的行为特征及其性伴HIV感染状况。方法调查2015-2017年天津市经男男同性间性传播的15~24岁HIV感染者/AIDS病例,内容包括个人基本信息、艾滋病知识知晓情况、教育和心理支持需求、性行为情况、性伴情况等。开展HIV阳性青年MSM当前性伴的问卷调查和HIV抗体检测。结果调查HIV阳性青年MSM共191人,被诊断HIV阳性时身份是学生的占48.69%(93人),获取艾滋病知识的主要途径为校园的占6.81%(13人),最近一年认识性伴的途径是交友软件的占75.92%(145人),将自身感染情况告知全部性伴的占比为40.31%(77人),HIV确证后性伴数量减少(χ~2=95.696,P<0.001)。调查了HIV阳性青年MSM的当前性伴93人,与固定性伴和临时性伴发生性行为坚持使用安全套的占比分别为22.58%(21/93)和37.63%(35/93),不知道性伴中有HIV感染者的占比为73.12%(68/93),被调查性伴的HIV感染率为11.83%(11/93)。结论 HIV阳性青年MSM主要通过交友软件寻找性伴,大多未将感染情况告知性伴,当前性伴安全套使用率低,HIV感染率较高,应该采取有效措施促进性伴告知,提高安全套使用率,加大校园艾滋病知识普及和警示教育。
Objective To understand the behavioral characteristics of HIV positive young MSM and HIV infection of their sexual partners in Tianjin. Methods The HIV/AIDS patients transmitted by homosexual behavior aged between 15-24 years were investigated in Tianjin during 2015 to 2017, to get the information on their basic personal characteristics, AIDS knowledge, needs of educational and psychological support, sexual behavior, and sexual partners, via the questionnaire and HIV antibody testing for the sexual partners. Results Out of the 191 HIV positive young MSM investigated, 48.69%(93) were students diagnosed as HIV positive. 6.81%(13) acquired AIDS knowledge in school. 75.92%(145) found sexual partners through dating software in the last year. 40.31%(77) informed all of their sexual partners about their HIV infection. The number of sexual partners reduced after infection(χ~2=95.696,P < 0.001). The HIV positive young MSM had a total of 93 current sexual partners. The condom use rates with fixed and temporary homosexual partners were 22.58%(21/93) and 37.63%(35/93). 73.12%(68/93) of them did not know that they had HIV positive sexual partners. The HIV infection rate of sexual partners was 11.83%(11/93). Conclusion The HIV positive young MSM seek sexual partners through dating software, and most of them do not inform their sexual partner about HIV infection. The condom use rate of current sexual partners is low, and the HIV infection rate is high. Effective measures should be taken to promote partner notification and condom use, and increase AIDS knowledge awareness of the college students.
