摘要
目的通过比较湿化氧疗和未湿化氧疗治疗老年慢性阻塞性肺疾病(COPD)效果的优劣性,探讨未湿化吸氧在老年慢阻肺患者中的可行性。方法选取上海市浦东医院呼吸内科2017年4月-2018年3月收治的需低流量吸氧治疗的COPD患者214例,随机分为湿化氧疗组(n=107)和未湿化氧疗组(n=107),分别给予常规湿化氧疗和未湿化氧疗,疗程为1周。观察并比较2组吸氧舒适度、护理操作时长及湿化瓶生物菌落计数等指标。结果2组在吸氧舒适度各参数上差异均无统计学意义(P>0.05);未湿化氧疗组的护理操作时长明显少于湿化氧疗组[(49.4±6.8)min vs(122±36.3)min,P<0.001],湿化瓶微生物菌落数上明显低于湿化氧疗组[(15.3±5.7)个vs(22.5±8.6)个,P<0.001)]。结论未湿化吸氧可以有效减少护理耗时,并可能对降低后期感染的发生率有益,相对于湿化氧疗,显示出操作便利优势和可能的低感染风险优势,同时并不降低患者的氧疗舒适度,可作为需要长期吸氧的患者备选方案。
Objective To compare the advantages and disadvantages of humidification oxygen therapy and non-humidification oxygen therapy in the treatment of chronic obstructive pulmonary disease(COPD)in the elderly,and to explore the feasibility of non humidification oxygen inhalation in the elderly patients with COPD.Methods A total of 214 patients with COPD treated with low-flow oxygen inhalation who were admitted to the Department of Respiratory Medicine,Pudong Hospital from April 2017 to March 2018 were enrolled,including 132 males and 82 females.They were divided randomly into the humidification oxygen therapy group(n=107)and the non-humidification oxygen therapy group(n=107),and were given conventional humidification oxygen therapy or non humidification oxygen therapy,respectively,for one week.The comfort level of oxygen absorption,nursing operation time and biological colony count in humidification bottle were observed and compared between the two groups.Results There was no significant difference in the parameters of comfort level of oxygen absorption during oxygen therapy between the two groups(P>0.05).Compared with the humidification group,the nursing operation time was significantly shortened[(49.4±6.8)min vs(122±36.3)min,P<0.001]and the counts of biological colonies in humidified bottles were significantly less[(15.3±5.7)vs(22.5±8.6),P<0.001)]in the non-humidification oxygen group.Conclusion Nonhumidification oxygen inhalation can effectively reduce nursing time and may be beneficial to reduce the incidence of late infection.Compared with humidification oxygen therapy,Non-humidified oxygen therapy shows the advantages of convenient operation and possible low infection risk,and does not reduce the patient’s oxygen therapy comfort.It can be used as an alternative for patients who need long-term oxygen inhalation.
