航天职工幽门螺杆菌感染与胃蛋白酶原、代谢指标的相关性研究 被引量:3

The study of Helicobacter pylori infection and the correlation with serum pepsinogen and indexes of metabolism in aerospace staff

摘要 目的分析北京地区航天系统职工幽门螺杆菌(H.pylori)的感染状况,以及与血清胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)、PGⅠ/PGⅡ(PGR)和代谢综合征(MS)各项指标的相关性。方法对2018年1-12月于航天中心医院健康管理部进行健康体检的4750名航天系统职工的体检资料进行回顾性分析。根据13碳-呼气试验(13CUBT)的检测结果,分析该人群H.pylori的感染特征,并将人员分为H.pylori阳性组和H.pylori阴性组,比较两组人员血清PGⅠ、PGⅡ、PGR和MS的诊断率及各项代谢指标:空腹血糖(FBG)、甘油三酯(TG)、总胆固醇(TC)、高密度胆固醇(HDL-C)、低密度胆固醇(LDL-C)、尿酸(UA)、体重指数(BMI)等水平的差异,分析H.pylori感染与上述指标的相关性。结果4750名航天系统职工H.pylori感染率为25.8%(1225/4750),MS诊断率为14.2%(673/4750)。H.pylori阳性组血清中PGR(4.89±1.52)和HDL-C(1.07±0.46)mmol/L水平低于阴性组(5.23±1.67)、(1.11±0.46)mmol/L,MS检出率(16.2%)及TG水平(1.53±1.46)mmol/L高于阴性组(13.5%)、(1.41±1.53)mmol/L,差异均有统计学意义(P均<0.05)。两组PGⅠ、PGⅡ、BMI、FBG、TC、LDL-C、UA比较差异均无统计学意义(P>0.05)。Pearson相关性分析显示,H.pylori感染与TG水平呈正相关(r=0.551,P<0.05),与PGR和HDL-C呈负相关(r=-0.542、-0.443,P<0.05)。结论幽门螺杆菌感染与血清胃蛋白酶原和代谢综合征有相关性,检测幽门螺杆菌感染对于胃部疾病和代谢性疾病的防治有重要意义。 Objective To analyze Helicobacter pylori(H.pylori)infection and the correlation with serum pepsinogenⅠ(PGⅠ),pepsinogenⅡ(PGⅡ)and PGⅠ/PGⅡ(PGR)and indexes of metabolic syndrome(MS)in aerospace staff in Beijing.Methods A retrospective study was conducted based on the data from 4750 aerospace staff.The prevalence of Helicobacter pylori infection was studied.According to the presence of H.pylori infection,the subjects were divided into H.pylori-positive group and H.pylori-negative group.The indexes of serum PGⅠ,PGⅡ,PGR and indexes of metabolism fasting blood glucose(FBG),triacylglycerol(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),uric acid(UA),body mass index(BMI)were compared between two groups.The correlation between H.pylori infection and the above indexes were analyzed.Results Among the 4750 subjects,1225 were H.pylori-positive and the positive rate was 25.8%;673 were diagnosed MS and the positive ratio was 14.2%.The levels of serum PGR and HDL-C were lower,while MS ratio and TG level were higher in H.pylori-positive group[(4.89±1.52),(1.07±0.46)mmol/L,16.2%,(1.53±1.46)mmol/L]than H.pylori-negative group[(5.23±1.67),(1.11±0.46)mmol/L,13.5%,(1.41±1.53)mmol/L](P<0.05).There was no significant difference in PGⅠ,PGⅡ,BMI,FBG,TC,LDL-C,UA between the two groups(P>0.05).Pearson correlation analysis showed that H.pylori infection was positively correlated with TG level(r=0.551,P<0.05),and negatively correlated with PGR and HDL-C(r=-0.542,-0.443,P<0.05).Conclusions H.pylori infection might affect serum pepsinogen and related with MS.Diagnosis,prevention and curing of H.pylori infection should be important in guaranteeing the health of aerospace staff.
