基于行为科学管理理论护理干预对神经源性膀胱手术患儿及陪护人员情绪依从性护理满意度的影响 被引量:6

摘要 目的探讨基于行为科学管理理论的护理干预对神经源性膀胱手术患儿及陪护人员情绪、依从性、护理满意度的影响.方法将87例神经源性膀胱手术患儿按入院建档顺序分为观察组(44例)与对照组(43例).两组均予以常规护理干预,观察组在此基础上实施基于行为科学管理理论的护理干预,观察住院全程.于干预前后采用儿童医疗恐惧量表、自制患儿抵触情绪调查表评估患儿的恐惧情绪及抵触情绪,采用汉密顿焦虑量表、汉密顿抑郁量表、患儿陪护人员依从性问卷调查表评定陪护人员的焦虑抑郁情绪及依从性.统计并发症发生状况及陪护人员的护理满意度.结果(1)干预后两组儿童医疗恐惧量表的自我恐惧、医疗环境恐惧、医疗操作恐惧因子分均较干预前显著降低(P<0.01),观察组显著低于对照组(P<0.01).(2)干预后两组患儿抵触情绪调查表的治疗依从性及患儿沟通意向因子分较干预前显著升高(P<0.01),观察组显著高于对照组(P<0.01),服药时抵触情绪评分较干预前显著降低(P<0.01),观察组显著低于照组(P<0.01).(3)干预后两组陪护人员汉密顿焦虑量表、汉密顿抑郁量表评分均较干预前显著降低(P<0.01),观察组显著低于照组(P<0.01).(4)观察组陪护人员依从率及护理满意率显著高于对照组(P<0.05或0.01).(5)两组并发症发生率比较差异无统计学意义(P>0.05).结论基于行为科学管理理论的护理干预能有效调节神经源性膀胱手术患儿的医疗恐惧及抵触情绪,改善陪护人员的焦虑抑郁情绪,增强陪护人员依从性,提高陪护人员护理满意度,减少并发症的发生. Objective To explore the effect of nursing intervention based on behavioral science management theory on emotion,compliance and nursing satisfaction of children and accompanying staff undergoing neurogenic bladder surgery.Methods 87 cases of NB children were divided into observation group(n=44)and control group(n=43)according to the order of admition filing.The control group was given routine nursing intervention,and the observation group was given nursing intervention based on behavioral science management theory.Observe the whole process of hospitalization.Before and after the intervention,the children's medical fear scale and self-made children's resistance mood questionnaire were used to evaluate the children's fear and resistance.HAMA,HAMD,and child caregiver compliance questionnaires were used to assess the anxiety,depression and compliance of the caregivers.The occurrence of complications and the satisfaction of nursing staff were analyzed.Results(1)After the intervention,the scores of self-fear,medical environment fear,and medical operation fear factors of the children's medical fear scale were significantly lower than those before the intervention(P<0.01),and the observation group was significantly lower than the control group(P<0.01).(2)After the intervention,the treatment compliance and the children's communication intention factor scores in the resistance mood of the two groups were significantly higher than those before the intervention(P<0.01).The conflict emotions score was significantly lower when taking the medicine than before the intervention(P<0.01),and the observation group was significantly lower than that of the control group(P<0.01).(3)The scores of HAMA and HAMD in the two groups were significantly lower than those before the intervention(P<0.01),and the observation group was significantly lower than the control group(P<0.01).(4)The compliance rate and satisfaction rate of nursing staff in the observation group were significantly higher than those in the control group(P<0.05 or 0.01).(5)There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions Nursing intervention based on behavioral science management theory applied in NB operation can regulate children's emotions,alleviate the negative emotions of the accompanying staff,enhance the compliance of the accompanying staff,improve the nursing satisfaction of the accompanying staff,and also have fewer complications.
