摘要
目的探讨吡美莫司乳膏联合308 nm准分子激光治疗面部白癜风患者的临床疗效及安全性.方法将135例面部白癜风患者按随机数字表法分为3组,各45例.试验组予以吡美莫司乳膏联合308 nm准分子激光治疗,药膏组予以吡美莫司乳膏治疗,光疗组予以308 nm准分子激光治疗,观察6个月.比较3组临床效果、复色开始时间、复色率、不良反应发生率,随访3个月统计复发率,于治疗前后评定病变皮肤色素及患者生活质量.结果(1)治疗3个月及6个月末试验组总有效率均显著高于药膏组及光疗组(P<0.01).(2)试验组复色开始时间显著早于药膏组及光疗组(P<0.01),复色率显著高于药膏组及光疗组(P<0.01).(3)治疗6个月末3组病变皮肤色素评分均较治疗前提高(P<0.01),试验组高于药膏组及光疗组(P<0.01);皮肤病生活质量指数量表评分较治疗前显著降低(P<0.01),试验组显著低于药膏组及光疗组(P<0.01).(4)3组不良反应发生率及随访3个月复发率比较差异无统计学意义(P>0.05).结论吡美莫司乳膏联合308 nm准分子激光治疗面部白癜风疗效显著,可促进病变皮肤复色,提高患者生活质量,安全性高.
Objective To observe the clinical effect of combined use of pimecrolimus cream and 308 nm excimer laser in the treatment of facial vitiligo.Methods A total of 135 patients with facial vitiligo were divided into 3 groups according to the random number table method,each with 45 patients.The experimental group was treated with pimecrolimus cream combined with 308 nm excimer laser.The ointment group was treated with pimecrolimus cream,and the phototherapy group was treated with 308 nm excimer laser and observed for 6 months.Compare the clinical effect,the start time of admixture colour,the rate of admixture colour,the incidence of adverse reactions,and the recurrence rate of the three groups.The skin pigmentation of patients and the quality of life of patients were evaluated before and after treatment.Results(1)After 3 months and 6 months of treatment,the total effective rate of the experimental group was significantly higher than that of the ointment group and the phototherapy group(P<0.01).(2)The start time of admixture colour in the experimental group was shorter than that in the ointment group and the phototherapy group(P<0.01).The admixture color rate was higher than that of the ointment group and the phototherapy group(P<0.01).(3)After 6 months of treatment,the skin pigmentation score of the lesions was higher than that before treatment(P<0.01),and the experimental group was higher than the ointment group and the phototherapy group(P<0.01).The DLQI score was lower than that before the treatment,and the experimental group was lower than the ointment group and the phototherapy group(P<0.01).(4)There was no significant difference between the incidence of adverse reactions and the recurrence rate at 3 months follow-up in the three groups(P>0.05).Conclusions The combination of pimecrolimus cream and 308 nm excimer laser treatment for facial vitiligo can improve the curative effect,promote the color separation of diseased skin,improve the quality of life of patients,and have high safety.
