摘要
目的探讨轻度抑郁症患者认知功能损害的临床特点及事件相关电位P300与轻度抑郁症的相关性。方法收集2016年6月~2018年6月在温州市中医院神经内科门诊及住院的轻度抑郁症患者40例(抑郁组)及健康体检者40例(对照组)。两组患者均使用简易精神状态量表(MMSE)及蒙特利尔认知评估量表(MoCA)进行认知功能的评定,同时进行事件相关电位P300(ERP/P300)的检测,分析两者的相关性。结果与对照组比较,抑郁组MMSE及MoCA评分均明显减低(P<0.01),且P300潜伏期明显延长(P<0.01)。抑郁组MMSE子项目中即刻记忆、计算力、短程记忆较对照组低(P<0.01)。MoCA子项目中执行功能、延迟回忆、注意力、语言较对照组低(P<0.01)。MMSE和MoCA呈正相关(r=0.846,P<0.01),MMSE和P300潜伏期呈负相关(r=-0.645,P<0.01),MoCA与P300潜伏期呈负相关(r=-0.537,P<0.01)。结论轻度抑郁症患者存在一定的认知功能损害,事件相关电位P300指标异常可反映抑郁症患者认知功能损害。
Objective To investigate the clinical features of cognitive impairment in patients with mild depression and the correlation between event-related potential P300 and mild depression.Methods 40 patients with mild depression(depression group)and 40 healthy subjects(control group)were enrolled in the neurology outpatient and inpatient and hospitalized in Wenzhou Hospital of Traditional Chinese Medicine from June 2016 to June 2018.Both groups of patients were assessed for cognitive function using the Mini Mental State Scale(MMSE)and the Montreal Cognitive Assessment Scale(MoCA),and the event-related potential P300(ERP/P300)was tested to analyze the correlation between the two.Results The MMSE and MoCA scores in the depression group were significantly lower than those of the control group(P<0.01),and the P300 latency in the depression group was significantly prolonged(P<0.01).The immediate memory,computational power,and short-course memory in the MMSE subproject of the depression group were lower than those in the control group(P<0.01).The executive function,delayed recall,attention,and language in the MoCA subproject were lower than those in the control group(P<0.01).MMSE and MoCA were positively correlated(r=0.846,P<0.01),MMSE and P300 were negatively correlated(r=-0.645,P<0.01),and MoCA was negatively correlated with P300 latency(r=-0.537,P<0.01).Conclusion There is a certain cognitive impairment in patients with mild depression.The abnormality of event-related potential P300 can reflect cognitive impairment in patients with depression.
