摘要
难治性肾病(Refractory Nephritic Syndrome,RNS)是儿科较为棘手的泌尿系统疾病,王雪峰教授基于对气与火的理解,根据《黄帝内经》中"少火生气,壮火食气"理论,对RNS的病机演变有了新的认识,提出清热益气养阴为治疗小儿难治性肾病的基本治疗大法,并逐步揭示RNS激素疗法中会依次出现"壮火(壮火内炽)""壮火食气"及"气食少火(少火不足)"三个阶段。其认为"壮火食气"是激素依赖型肾病及频复发型肾病的病机关键,而维持机体"少火生气"是本病的治疗重点。在临床中要综合分析RNS的病程及病情变化规律,运用"少火生气"这一理念引导辨证论治、立法处方以减轻激素的副作用及防止RNS复发。
Pediatric refractory nephritic syndrome(RNS)is a more difficult urinary tract disease in pediatrics,professor WANG Xuefeng based on understanding of gas and fire,according to Inner Canon of Huangdi of"strong fire consuming Qi"theory,a new understanding of pathogenesis of RNS evolution,the basic treatment of RNS by heat clearing,Qi nourishing and Yin nourishing were proposed,the three stages of"strong fire""strong fire consuming Qi"and"Qi consuming junior fire"were revealed in the RNS hormone therapy. Professor WANG Xuefeng believes that"strong fire consuming Qi"is the key to the pathogenesis of steroid dependent nephropathy syndrome and frequently relapsing nephritic syndrome,and maintain the body’s"junior fire supporting vital Qi"is the treatment of the disease. To comprehensive analysis of RNS in clinical course of the disease and illness change rule,using the"junior fire supporting vital Qi"this concept to guide treatment based on syndrome differentiation,legislative prescription in order to reduce the side effect of hormone and prevent the recurrence of RNS.
