摘要
目的探讨宫颈细胞DNA倍体分析在宫颈癌前筛查中的应用价值。方法选取2017年4月~2019年6月在我院进行宫颈癌前筛查的473例已婚妇女,均行宫颈细胞学检查、HPV检测、宫颈细胞DNA倍体分析及宫颈活检组织病理检测。分析宫颈细胞DNA倍体分析在宫颈癌前筛查中的诊断价值。结果473例入选者中,TCT≥ASCUS者94例,HPV(+)者102例,宫颈细胞DNA倍体分析结果阳性者85例,宫颈活检组织病理检测阳性者91例。宫颈细胞DNA倍体分析的符合率、敏感度及特异性均高于宫颈细胞学检测、HPV检测(P<0.05)。宫颈细胞学检测的符合率、敏感度及特异性均高于HPV检测(P<0.05)。宫颈细胞学与HPV双阴性者宫颈细胞DNA倍体分析的符合率为99.67%,宫颈细胞学TCT≥ASCUS但HPV阴性者宫颈细胞DNA倍体分析的符合率为96.23%,差异无统计学意义(P>0.05)。对正常宫颈者和低度癌前病变者随访3个月,宫颈细胞DNA倍体分析的符合率高于宫颈细胞学检测、HPV检测(P<0.05)。宫颈细胞学检测的符合率高于HPV检测(P<0.05)。结论宫颈细胞DNA倍体分析在宫颈癌前筛查中与宫颈组织活检具有较高的符合率,能在现有宫颈癌筛查方式的基础上尽可能地提高准确率,减少漏诊率,且敏感度及特异性均较高。
Objective To investigate the application value of DNA ploidy analysis on cervical cells in cervical precancerous screening.Methods A total of 473 married women who were given cervical precancerous screening in our hospital from April 2017 to June 2019 were selected for cervical cytology,HPV testing,DNA ploidy analysis of cervical cells,and histopathological examination of cervical biopsy.The diagnostic value of cervical cell DNA ploidy analysis in cervical precancerous screening was analyzed.Results Among the 473 enrolled cases,94 cases were TCT≥ASCUS,102 cases were HPV(+),85 cases were positive for cervical cell DNA ploidy analysis,and 91 cases were positive for pathological examination of cervical biopsy.The coincidence rate,sensitivity,and specificity of cervical cell DNA ploidy analysis were higher than those in cervical cytology and HPV test(P<0.05).The coincidence rate,sensitivity and specificity of cervical cytology were higher than those of HPV test(P<0.05).The coincidence rate of cervical cytology and DNA ploidy analysis of cervical cells in HPV double negative patients was 99.67%.The coincidence rate of DNA ploidy analysis in cervical cytology with TCT≥ASCUS but HPV negative was 96.23%,and the difference was not statistically significant(P>0.05).Patients with normal cervix and low precancerous lesions were followed up for 3 months.The coincidence rate of cervical cell DNA ploidy analysis was higher than that of cervical cytology and HPV test(P<0.05).The coincidence rate of cervical cytology was higher than that of HPV test(P<0.05).Conclusion The DNA ploidy analysis on cervical cells has a high coincidence rate with cervical biopsy in cervical precancerous screening,which can improve the accuracy rate and reduce the rate of missed diagnosis as much as possible based on the existing cervical cancer screening methods,and the sensitivity and specificity are high.
