摘要
目的探讨体质量法和年龄法两种剂量计算方法对儿童青霉素类药物处方前置审核拦截率的影响,为处方前置审核系统拟定拦截标准提供参考和依据。方法收集本院门诊2018年7月至12月的儿童患者静脉用青霉素类药物处方1098张,根据药品说明书以及《中国医师药师临床用药指南》以及相关文献制定按体质量和年龄两种计算方法的剂量标准,模拟处方前置审核,统计出按两种剂量计算方法进行剂量审核的拦截率,采用配对计数资料的卡方检验(McNemar检验)判断两种剂量计算方法的拦截率有无差异。结果青霉素处方按体质量法拦截率为10.7%,按年龄法拦截率为0.0%,差异有统计学意义(P<0.05);阿莫西林舒巴坦处方按体质量法拦截率为26.4%,按年龄法拦截率为55.4%,差异有统计学意义(P<0.05);美洛西林舒巴坦处方按体质量法拦截率为24.7%,按年龄法拦截率为6.7%,差异有统计学意义(P<0.05);哌拉西林舒巴坦处方按体质量法拦截率为25.7%,按年龄法拦截率为43.7%,差异有统计学意义(P<0.05)。结论选择体质量法计算儿童剂量对青霉素类药物处方的拦截率适中,较按年龄折算法更加准确,因此体质量法可作为首选,须设定不超过成人剂量的最高剂量限制;当患儿肥胖或消瘦时,按体质量计算剂量的同时应根据年龄进行调整。
Objective To explore the influence of dose calculation method according to weight or age on the interception rate of pre-prescription review of penicillins prescriptions in children,and to provide references and basis for formulating interception standard for pre-prescription review system of penicillins prescriptions.Methods A total of 1098 intravenous penicillins prescriptions for children in our outpatient department from July to December,2018 were collected.In terms of the drug instructions,the Guidelines for Clinical Drug Use by Chinese Physicians and Pharmacists,and related literature,the dosage standards were formulated according to body weight and age.Pre-prescription verification was simulated.And the interception rates of dose auditing by the two methods were calculated.The difference in the interception rate between the two methods was judged by the Chi-square test of paired counting data(McNemar test).Results The interception rate of penicillin prescription by the weight method was 10.7%,and that by the age method was 0.0%,with statistical difference(P<0.05).The interception rate of amoxicillin sulbactam prescription by weight method was 26.4%,and that of age method was 55.4%.The difference was statistically significant(P<0.05).The interception rate of mezlocillin sulbactam prescription by the weight method was 24.7%,and that by the age method was 6.7%,with a statistical difference(P<0.05).The interception rate of piperacillin sulbactam prescription by the weight method was 25.7%,and that by the age method was 43.7%,with statistical difference(P<0.05).Conclusion Weight-based method is more accurate than age-based method in calculating the interception rate of penicillin prescriptions in children,so the weight method can be the first choice.A maximum dose limit not exceeding adult doses should be set.If children are obese or thin,the dosage should be adjusted according to age while calculating the dosage according to body weight.
