摘要
目的探讨腰硬膜外阻滞结合功能锻炼治疗腰椎间盘突出症患者疼痛的疗效分析。方法选取本院2017年1月至2018年1月期间收治的80例腰椎间盘突出症患者作为研究对象,按照随机数字表的方法分为两组,各40例。对照组实施常规治疗,观察组则在对照组基础上采取腰硬膜外阻滞结合功能锻炼治疗,分别对比两组患者治疗前后NPQ(Northwick Park)量表评分、视觉模拟评分表(Vas)、JOA(日本骨科协会腰痛评分表)以及生活质量评分。结果治疗前,两组患者NPQ、VAS量表评分比较,差异均无统计学意义(均P>0.05);治疗后,观察组NPQ、VAS量表评分分别为(32.39±6.49)分、(3.38±0.88)分,均低于对照组,差异均有统计学意义(均P<0.05);观察组主观症状、临床体征、日常活动受限度分别为(7.85±1.39)分、(6.01±1.32)分、(8.42±2.58)分,均高于对照组,差异均有统计学意义(均P<0.05);观察组生活质量各评分均优于对照组,差异均有统计学意义(均P<0.05)。结论腰硬膜外阻滞结合功能锻炼治疗可以有效改善腰椎间盘突出症患者的疼痛状态,且在一定程度上提高了患者的预后生活质量,故具推广优势。
Objective To explore the effect of lumbar epidural block combined with functional exercise in the treatment of pain in patients with lumbar disc herniation.Methods 80 patients with lumbar disc herniation admitted to our hospital from January,2017 to January,2018 were selected as research objects,and were divided into an observation group and a control group according to the random number table,with 40 cases in each group.The control group received routine treatment;in addition,the observation group took lumbar epidural block and did functional exercise.The scores of NPQ(Northwick Park)scale,visual analogue scale(VAS),JOA(Japanese Orthopedic Association Rating Scale for Lumbago)and quality of life before and after the treatment in both groups were respectively analyzed.Results Before the treatment,there were no statistical differences in the scores of NPQ and VAS between the two groups(both P>0.05).After the treatment,the scores of NPQ and VAS were(32.39±6.49)and(3.38±0.88)in the observation group,which were significantly lower than those in the control group(both P<0.05).The scores of subjective symptoms,clinical symptoms,and daily activity limit were(7.85±1.39),(6.01±1.32),and(8.42±2.58)in the observation group,which were better than those in the control group(all P<0.05).The scores of life quality were better in the observation group than in the control group(all P<0.05).Conclusion Lumbar epidural block combined with functional exercise therapy can effectively improve the pain of patients with lumbar disc herniation,and improve their prognosis and quality of life to a certain extent,so it has the promotion advantage.
