摘要
目的分析临床分离西弗射盾子囊霉菌的生物学特征及药物敏感性数据,为实验室的准确鉴定及临床治疗提供用药依据。方法收集2010年至2019年中山大学附属第一医院及中山市三乡医院分离到的西弗射盾子囊霉菌47株,观察并记录其在沙氏平板及科马嘉显色培养基上的菌落形态和镜下形态。从NCBI数据库下载西弗射盾子囊霉菌及临床常见念珠菌的ITS序列,获取的序列通过MEGA 7.0.26采用Neighbor-Joining法构建进化树,对西弗射盾子囊霉菌与临床念珠菌的亲缘关系进行分析。所有的菌株使用法国梅里埃公司的VITEK 2全自动细菌鉴定及药敏分析仪进行鉴定;采用法国梅里埃公司ATB-FUNG3真菌药敏检测试剂条检测其对5-氟胞嘧啶、两性霉素B、氟康唑、伊曲康唑和伏立康唑的药物敏感性。结果西弗射盾子囊霉菌落干燥、白色、镶嵌生长;在科马嘉显色培养基呈现蓝色。系统进化分析显示西弗射盾子囊霉与酿酒酵母菌的亲缘关系较近。药敏结果显示菌株对于氟康唑具有较高的MIC,耐药率为70.2%;对于5-氟胞嘧啶的MIC分布范围为MIC50为4.0μg/ml,MIC90为≥32.0μg/ml;两性霉素B的MIC50为0.5μg/ml,MIC90为1.0μg/ml;伊曲康唑的MIC50为0.25μg/ml,MIC90为1.0μg/ml;伏立康唑的MIC50为0.5μg/ml,MIC90为1.0μg/ml。结论西弗射盾子囊霉菌对于抗真菌药物具有较高的耐药性,微生物实验室应重视真菌的培养鉴定和药敏实验,以便临床医生尽早、准确、合理的选用抗生素。医院感染管理科需要加强该菌的医院感染控制工作。
Objective To analyze the biological characteristics and the sensitivity to anti-fungal drugs of S.ciferrii and to provide some references for the accurate identification and treatment.Methods 47 strains of S.ciferrii were collected from First Hospital,Sun Yat-Sen University and Sanxiang Hospital.All the strains were identified by VITEK 2 system.The phylogenetic tree constructed by ITS sequence was used to analyze the phylogenetic relationship between S.ciferrii and other common clinical candida.All of the ITS sequences were downloaded from the NCBI database.The phylogenetic tree was constructed using Neighbor-Joining method by MEGA 7.0.The susceptibilities of 5-fluorocytosine,amphotericin B,fluconazole,itraconazole,and voriconazole were tested by the ATB-FUNG3.Results The colony of S.ciferrii was creamy,rough,and wrinkled after 1 week’s incubation.The colony appeared blue on CHRMagar medium.The phylogenetic relationship analysis showed that the relationship between S.ciferrii and Saccharomyces cerevisiae was closer than that of other common Candidas.The results of anti-fungal drugs sensitivity showed the strains had high resistance against fluconazole,with a drugresistant rate of 70.2%.The MIC50 of 5-fluorocytosine was 4.0μg/ml,and the MIC90≥32.0μg/ml.The MIC50 of amphotericin B was 0.5μg/ml,and the MIC901.0μg/ml.The MIC50 of itraconazole was 0.25μg/ml,and the MIC901.0μg/ml.The MIC50 of voriconazole was 0.5μg/ml.and the MIC901.0μg/ml.Conclusions S.ciferrii showed high resistance against antifungal drugs.In order to select antibiotics accurately and reasonably,clinical laboratory staff should pay attention to the cultivation,identification,and susceptibility test of S.ciferrii.The hospital infection control department also should pay attention to the monitoring of S.ciferrii.
