摘要
目的评价线阵超声内镜(LEUS)检查对远端胆管癌术前T分期诊断的准确性。方法拟行腹腔镜胰十二指肠切除术的远端胆管癌患者56例,术前对其行LEUS检查,进行术前T分期,以术后病理T分期作金标准,计算LEUS检查对远端胆管癌术前T分期的灵敏度、特异度、准确度,对LEUS检查术前T分期结果与术后病理T分期结果作一致性检验即Kappa检验。结果56例远端胆管癌术后病理T分期分别为T1期7例、T2期23例、T3期26例,LEUS检查远端胆管癌术前T分期分别为T1期6例、T2期23例、T3期27例。LEUS检查对远端胆管癌术前T分期诊断的灵敏度:T1期为85.7%,T2期为87.0%,T3期为92.3%;特异度:T1期为100.00%,T2期为90.9%,T3期为90.0%;准确度:T1期为98.2%,T2期为89.3%,T3期为91.1%。LEUS检查术前T分期结果与术后病理T分期结果Kappa检验的Kappa值(Kappa值>0.8为一致性极高,Kappa值0.8->0.6为一致性较高):T1期0.913,T2期0.779,T3期0.821。结论LEUS检查对远端胆管癌术前T分期诊断的准确性较高,与术后病理T分期有较高的一致性。
Objective To evaluate the accuracy of linear endoscopic ultrasound(LEUS)examination for T stage of distal cholangiocarcinoma before surgery.Methods Fifty-six patients with distal cholangiocarcinoma who planned to undergo laparoscopic pancreaticoduodenectomy were examined for LEUS before surgery as well as preoperative T staging.After taking the postoperative pathological T stage as the gold standard,we calculated the sensitivity,specificity,and accuracy of LEUS examination for pre-operative T staging of distal cholangiocarcinoma.Consistency test,that is,Kappa test,was performed on the pre-operative T staging results of LEUS examination and postoperative pathological T staging results.Results Of 56 cases of distal cholangiocarcinoma postoperative T staging,7 cases were found in stage T1,23 cases in stage T2,and 26 cases in stage T3.As for the preoperative LEUS T staging,6 cases were found in stage T1,23 cases in stage T2,and 27 stages in stage T3.The sensitivity of the LEUS examination in T staging of distal cholangiocarcinoma before surgery:85.7%in stage T1,87.0%in stage T2,and 92.3%in stage T3;specificity:100.00%in stage T1,90.9%in stage T2,and T3 in stage T390.0%;accuracy:98.2%in T1,89.3%in T2,and 91.1%in T3.The results of Kappa test(Kappa value>0.8 is extremely consistent,and Kappa value 0.8->0.6 is highly consistent)were:stage T10.913,stage T20.779,and stage T30.821.Conclusion The accuracy of the LEUS test for T staging of distal cholangiocarcinoma is high,and it is consistent with the result of postoperative pathological T staging.
