摘要
目的系统分析重症新型冠状病毒肺炎(COVID-19)患者基础疾病的患病率。方法计算机检索PubMed、EMbase、中国知网(CNKI)、维普网(VIP)数据库和万方数据知识服务平台,查找与重症COVID-19患者合并基础病相关的文献,检索时限为2019年12月—2020年4月13日。由2名研究员独立完成文献筛选、数据收集并评价纳入研究的偏倚风险,采用Stata12.0软件进行Meta分析。结果共纳入40项回顾性研究,共计4261例重症COVID-19患者,均为高质量研究。Meta分析结果显示:重症COVID-19患者最常见的基础疾病是高血压[37.3%,95%CI(33.7%~41.1%)],其次为糖尿病[19.3%,95%CI(16.7%~21.9%)]、心血管疾病[15.4%,95%CI(12.9%~18.1%)];部分患者合并脑血管疾病[10.8%,95%CI(7.6%~14.5%)]、慢性阻塞性肺病(COPD)[9.5%,95%CI(7.1%~12.2%)],少数患者可伴有慢性肾病[5.5%,95%CI(3.7%~7.5%)]、恶性肿瘤[5.0%,95%CI(4.2%~5.7%)]及慢性肝病[4.5%,95%CI(3.0%~6.4%)]。结论当前证据表明,高血压、糖尿病和心脑血管疾病是重症COVID-19患者最常见的基础疾病;部分患者合并有COPD、恶性肿瘤、慢性肝病和慢性肾病。
Objective To systematically analyze the prevalence of underlying diseases in patients with severe coronavirus disease 2019(COVID-19).Methods PubMed,Embase,CNKI,VIP database,and Wanfang Data Knowledge Service Platform were electronically searched to collect studies about the prevalence of underlying diseases in severe COVID-19 patients from 1 December 2019 to 13 April 2020.Two reviewers independently screened literature,extracted data,and assessed the risk of bias of included studies,then,a meta-analysis was performed by using Stata12.0 software.Results A total of 40 retrospective studies involving 4261 severe COVID-19 patients were included.All of the studies were of high quality.Meta-analysis showed that the most common underlying disease was hypertension(37.3%,95%CI 33.7%to 41.1%),followed by diabetes(19.3%,95%CI 16.7%to 21.9%)and cardiovascular disease(15.4%,95%CI 12.9%to 18.1%).Other underlying diseases were cerebrovascular disease(10.8%,95%CI 7.6%to 14.5%),chronic obstructive pulmonary disease(COPD)(9.5%,95%CI 7.1%to 12.2%),chronic kidney disease(5.5%,95%CI 3.7%to7.5%),malignant tumors(5.0%,95%CI 4.2%to 5.7%)and chronic liver disease(4.5%,95%CI 3.0%to 6.4%).Conclusion Current evidence shows that hypertension,diabetes and cardio-cerebrovascular disease were the most common underlying diseases in severe COVID-19 patients.COPD,malignant tumors,chronic kidney disease,and chronic kidney disease were also observed in some patients.
