Advances on IHH signal transduction and cartilage metabolism in patients with osteoarthritis

摘要 骨关节炎(osteoarthritis,OA)是关节软骨的局部损伤和丢失、异常重塑和磨损、局部炎症等一系列病理改变导致的退行性变[1]。关节软骨是覆盖于关节的表面的一层透明软骨,大约由5%软骨细胞和95%基质构成,基质以蛋白多糖凝胶以及II型胶原为主,有着承受机械负荷、润滑关节、防止磨损等重要作用[2]。 Osteoarthritis (OA) is a degenerative change caused by local injury and loss,abnormal remodeling,or wear of the articular cartilage.Indian hedgehog (IHH),a member of the Hedgehog family,is mainly produced and secreted by prehypertrophic chondrocytes to regulate chondrocyte hypertrophy and endochondral bone formation during the growth plate development.It has been a hot and difficult research topic to find biomarkers and potential therapeutic targets related to the diagnosis of OA in recent years,while a large number of studies have confirmed that IHH is highly correlated with the articular cartilage metabolism.Articular cartilage presents a differentiation process similar to that of the endochondral osteogenesis in the development of OA:chondrocyte hypertrophy,terminal differentiation,ossification,apoptosis.The expression of IHH,related to the morphological transformation of the cartilage,is up-regulated in this process.Some studies have confirmed that the expression of IHH is up-regulated in the serum,synovial fluid and cartilage of OA patients.Some animal experiments have found that knocking out of the IHH gene in mice can delay the progress of OA,while others have noted that transplant of the vector encoding IHH into the cartilage damaged area can promote the cartilage repair.It is undeniable that IHH plays an important role in the regulation of cartilage metabolism in patients with OA,but the specific mechanism has not been fully clarified yet.At present,it has been found that the main regulatory mechanisms include IHH-Gli signaling pathway,IHH-PTHrP negative feedback loop,induction of BMSCs,etc.This paper reviews the research progress of IHH signal transduction and cartilage metabolism in patients with OA.
