下睑外翻动物模型的建立与评价

Establishment and evaluation of rabbit model with lower eyelid ectropion

摘要 目的评估经去除组织联合缝线法建立兔眼下睑外翻模型的可行性和有效性。方法日本健康大耳白兔6只。每只兔随机选取一只眼行下睑外翻手术(实验组),以对侧眼作自身空白对照(对照组)。分别于术后1、3、5、7、15、30 d测量结膜外翻、睑板下降程度,并观察伤口愈合和眼内分泌物情况,术后30 d检查实验组兔眼有无干眼症、结膜炎和眼球坏死等并发症,切除下睑组织行病理检查。结果结膜外翻和睑板下降程度:实验组与对照组相比,术后1~3 d结膜外翻加重、睑板下降明显,于术后3 d达到最大值,同时结膜出现充血,眼内分泌物增加;术后3~7 d,下睑组织逐渐回缩,结膜和睑板出现不同程度回升;术后15~30 d睑外翻逐渐稳定;测得数据两组间差异均具有统计学意义(P<0.05)。所有兔眼均愈合良好;实验组兔眼均出现不同程度的并发症,经组织病理学检查示眼睑腺体细胞排列紊乱,部分出现断裂;对照组眼睑细胞连续整齐。结论通过去除组织联合缝线法建立兔眼下睑外翻动物模型,操作简单、外翻明显、效果稳定、并发症较少、成本低廉,是一种值得推广的中、重度睑外翻模型制作的方法。 Objective To explore the feasibility and effectiveness oftissue removal combined with suture in the establishment of rabbit model with lower eyelid ectropion.Methods There were 6 Japanese big-ear rabbits in this study.One eye was randomly selected in each rabbit to establish the lower eyelid ectropion model(experimental group)and the other eye was served as self-control(control group).The conjunctival ectropion and decline degree of tarsal plate at 1,3,5,7,15,30 days after operation were measured and wound healing and intraocular secretions were observed.Complications such as xerophthalmia,conjunctivitis and ocular necrosis in the experimental group were examined at 30 days after operation.The lower eyelid tissue was removed for histopathological examination.Results Compared with the control group,conjunctival ectropion was aggravated and tarsal plate decline was obvious at 1 to 3 days after operation.The maximum value was reached at 3 days after operation.Meanwhile,the conjunctival congestion was appeared and intraocular secretions was increased.At 3 to7 days after operation,the lower eyelid tissue was gradually retracted and conjunctiva and tarsal plate were recovered in varying degrees.The eyelid ectropion was gradually stable at 15 to 30 days after operation.There was statistically significant between 2 groups(P<0.05).All rabbit eyes healed well.Varying degrees of complications occurred in the experimental group.The histopathological examination results showed that the gland cells of eyelids were disordered and some of them were broken,while the control group were continuous and orderly.Conclusion The rabbit model with lower eyelid ectropion could be established by tissue removal combined with suture.This method had simple operation,obvious ectropion,stable efficacy,less complications and lowcost.It should be considered for popularization in clinic.
